Dr. Mario Nakamatsu, also simply called Dr. Nakamatsu, is a scientist and medical doctor that shares an uncannly resemblance to Dr. Mario, but he is no member of the Mario family: he is named after a descendant of his from the Edo period, Mario Gennai; while his profession and surname are a nod to Yoshiro Nakamatsu. He appears as a recurring character in the Super Mario Kodansha manga.
As previously stated, he is identical to Dr. Mario, but has grey hair, and his head mirror has the word "Dr" written on it. He loves to create machines to help people, to the point of getting fired up when making them, but he also opposes the development of machines that can harm or manipulate the human will. He is the grandfather of Kinoppe, which also connects him as the father (or father-in-law) of Dr. Mario.
He first appears in the "Time Traveler" storyline, presented as the mind behind the Sky Pop, Marine Pop, and Mecha Kuribō. He invents a time machine that Bowser steals, so Dr. Nakamatsu gives Mario the prototype to chase him through time ans space, and a bag of items to use in case of emergency. During the story, Mario even meets up with the doctor's ancestor Mario Gennai. In the end, Mario resorts in landing back to the present, moments before the theft, and land on the time machines, stopping Bowser from altering history.
In Super Mario: Yoshi no Road Hunting 1, Bowser captures Dr. Nakamatsu and Kinoppe, and by threatening to cook her, forces the doctor to create battle mechas for him and his kids, so they can take over Jewelry Land. After his conquest, Bowser then forces Dr. Nakamatsu to create a machine to force Peach to love him. Instead, Dr. Nakamatsu creates the Super Scope and sends it to Mario through Mecha Patakuri, also attaching a video message that informs him that the battle mechas have hidden weak points he made sure to create. When Bowser discovers this, he threatens to cook Kinoppe, causing the doctor to faint, so he begins to formulate less scary threats to keep using the doctor with the help of Wendy.
In Super Mario: Yoshi no Road Hunting 2, Bowser orders the doctor to create the Kame Hōdai, but he refuses. Bowser, having found the perfect threat, tells him he will turn Kinoppe's face into Wendy's. Worried, Dr. Nakamatsu obliges, to the point of becoming so agitated that when Wendy frees Kinoppe, offended by the threat, the doctor doesn't acknowledge his granddaughter's escape. He even controls the Kame Hōdai during its second phase, only to finally realize that Kinoppe is free, stalling enough for Mario to blow up the tank and save the doctor from the explosion, who has since regained his composure. He then creates the Ultra Super Scope for the final battle and a remote-controlled armor for Yoshi that he attempts to control, ultimately causing him to tumble into a pit. When Bowser is finally defeated, Dr. Nakamatsu and Kinoppe return home, ready to invent something else.
