Dr. Mario Nakamatsu, also simply called Dr. Nakamatsu, is a scientist and medical doctor that shares an uncannly resemblance to Dr. Mario, but he is no member of the Mario family: he is named after a descendant of his from the Edo period,「毬夫源内」(Mariotto Gennai); while his profession and surname are a nod to Yoshiro Nakamatsu. He appears as a recurring character in the Super Mario Kodansha manga.
As previously stated, he is identical to Dr. Mario, but, in the Yoshi's Safari adaptation, he has clear-colored hair, and his head mirror has the word "Dr" written on it. He loves to create machines to help people, to the point of getting fired up when making them, but he also opposes the development of machines that can harm or manipulate individuals' will. When he is not creating stuff, he can be seen working as the head doctor at the Mushroom Kingdom Hospital with Nurse Peach, or spending time with his granddaughter Kinoppe (though it is unclear whether they are biologically related or if he adopted her).
He first appears in the Super Mario World 9 story arc (that was never made into a volume) as the inventor of a time machine that Bowser steals, to which the Doctor calls Mario to help him prevent Bowser from damaging the timeline.
He reappears in the Super Mario Kart arc. In Super Mario Kart 1, he is working at the hospital and takes care of the injured Donkey Kong Jr., after Mario brings him there, following a kart accident. He later comes to Mario's victory ceremony to request reimbursement for Junior's hospital bill.
In Super Mario Kart 3, during the Super Mario Cart Rally, Luigi gets badly injured, so Mario and Peach rush to the hospital. In doing so, their team takes the lead, and the rest of the racers follow them to the hospital, driving through the building's halls. After Dr. Nakamatsu tends to Luigi, the Mario team speeds away, just as the doctor was pulling out the hospital bill, thus getting run over by the rest of the racers who were still following Mario. Due to the damages, the medical board sends Dr. Nakamatsu and Nurse Peach to an isolated clinic by the Safari Course. There, the Mario team has to visit the doctor once again after Luigi is stung by a Kurayami Sasori. After revealing that Luigi is close to being killed by the poison, the Doctor reveals that the Amazons of Jungle Course should know where the antidote is. Once again speeding away right as the doctor pulls out the bill, the Mario team accidentally lures the rival racers to follow them, running over the clinic. Dr. Nakamatsu gets fired up and decides to join the rally so he can win the money prize to pay for the damages, driving with Nurse Peach in a kart shaped like Mario's head. While chasing the pilot, the doctor crashes into Bowser and Wario's kart. To hinder Mario's quest to save his brother, Wario takes the place of the doctor (while Nurse Peach is presumably out cold from the impact), and Bowser carries off the incapacitated doctor. While Wario nearly succeeds in preventing Luigi from being cured, due to Cannon Ball Z's intervention, Bowser loses his kart and decides to reach the finish line on foot, while carrying Cannon Ball Z (who was just revealed to be the Mushroom King) on his back, and making the doctor carry Cannon Boy z (Toad) on his. Unfortunately, having lost their vehicle, the pilots are disqualified, and nobody gets the prize.
In Super Mario: Yoshi no Road Hunting 1, where his white-haired design debuts, Bowser captures Dr. Nakamatsu and Kinoppe, and by threatening to cook her, forces the doctor to create battle mechas for him and his kids, so they can take over Jewelry Land. After his conquest, Bowser then forces Dr. Nakamatsu to create a machine to force Peach to love him. Instead, Dr. Nakamatsu creates the Super Scope and sends it to Mario through Mecha Patakuri, also attaching a video message that informs him that the battle mechas have hidden weak points he made sure to create. When Bowser discovers this, he assures he will cook Kinoppe, causing the doctor to faint, so he begins to formulate less scary threats to keep using the doctor with the help of Wendy.
In Super Mario: Yoshi no Road Hunting 2, Bowser orders the doctor to create the Kame Hōdai, but he refuses. Bowser, having found the perfect threat, tells him he will turn Kinoppe's face into Wendy's. Worried, Dr. Nakamatsu obliges, to the point of becoming so agitated that when Wendy frees Kinoppe, offended by the threat, the doctor doesn't acknowledge his granddaughter's escape. He even controls the Kame Hōdai during its second phase, only to finally realize that Kinoppe is free, stalling enough for Mario to blow up the tank and save the doctor from the explosion, who has since regained his composure. He then creates the Ultra Super Scope for the final battle and a remote-controlled armor for Yoshi that he attempts to control, ultimately causing him to tumble into a pit. When Bowser is finally defeated, Dr. Nakamatsu and Kinoppe return home, ready to invent something else.
In Super Mario: Yoshi Island 3, set after the events of the game, Dr. Nakamatsu (back to having his normal Dr. Mario design) witnesses as a Harry Hedgehog sting his granddaughter. To expel the poison from her blood, the doctor needs an immediate transfusion, but they live in the Mushroom Kingdom countryside. While Kinopio-san goes to deliver the rare blood type via bullet train, Baby Bowser attacks it in order to kidnap Baby Peach, who was on board. The Yoshi Help Team intervenes, saves Baby Peach, and delivers the blood to the doctor just in the nick of time.
