Hefty Mildes return alongside their smaller counterpart in Yoshi and the Mysterious Book after a twelve-year absence from the series. They appear in Milde Habitat and Short-to-Tall Roll Call.

When Yoshi falls into a Milde pool, he will reemerge after a few seconds, trapped within a transparent Hefty Milde. Yoshi can escape by moving the joystick, causing the Hefty Milde to burst. Hefty Mildes containing other creatures will also spawn if a small creature such as a Crazee Dayzee or Bounseed falls into a Milde pool, and opaque Hefty Mildes containing nothing will spawn once six standard Mildes are burst in close proximity to a Milde pool or if Yoshi bursts a Big Milde. Once spawned, Hefty Mildes walk around aimlessly and can be stomped on, bursting into six Mildes if empty and five Mildes plus its contents if not. Mildes thrown at a Hefty Milde will stick to it.

When attempting to eat a Hefty Milde, Yoshi's tongue will bounce off and cause it to ripple slightly. It will then walk in the direction it is facing for a few seconds. If forced to walk into water using this method, it will burst once deep enough.

Similar to Big Mildes and Mega Mildes, Hefty Mildes can have Scatterpuffs grown on them as though they were a solid surface; Hefty Mildes spawned by a parasitized creature falling into a Milde pool will spawn with a Scatterpuff cluster grown on them already.
