Frenzy mode (known in the "Tips & Tricks" as an item frenzy) is a state in Mario Kart Tour that the player enters when they have three of the same items. The player cannot receive a Frenzy from the first Item Box after their previous Frenzy. For the duration of a Frenzy, the player is invincible, they can use an indefinite amount of the acquired item, and, for most items, six of that item orbits around them. The orbiting items act as a timer, and vanish when the Frenzy ends. When the player uses an item during a Frenzy, they gain points. The player can also perform Ultra Mini-Turbos much faster during a Frenzy. Only one player can be in a Frenzy at the same time. The higher the driver's level and rarity is, the higher their chance of getting an Item Frenzy.

Items that last for a certain period of time, such as Bullet Bills, start the Frenzy used and only give extra points when reused during a Frenzy. Banana Barrels, Mushroom Cannon, Coin Box, and Bob-omb Cannon Frenzies are surrounded by the item they fire. Triple or double item Frenzies only have the singular version present. Bubbles do not have anything orbiting the players. Bloopers only attack one player in front of the player with each use. Bullet Bills have a smaller Bullet Bill on each side of the player which can be fired when used, and explode after touching the ground. Fire Flowers only fire one fireball at a time. Lightning only hits one player with each use. Spiny Shells can target other players and do not orbit around players before hitting them. Hearts has five hearts orbiting the players. The Lucky Seven has all of its seven items orbiting the players.

Certain Tour Challenges require the player to start a Frenzy.
