Artwork of Dr. Bowser from Dr. Mario World Dr. Bowser Dr. Mario World Dr. Bowser is Bowser's persona in the Dr. Mario series, first appearing in Dr. Mario World. His attire is somewhat reminiscent of his normal self's wedding attire from Super Mario Odyssey, which is made evident by similar shoes. He carries a stethoscope with his insignia on it around the back of his neck, though it appears to be too small for him to use. He appears in stage 3 in stage mode, where he decided to aid Dr. Mario and Dr. Peach in eliminating the viruses, mostly due to his own minions being affected by them. He is available as a special stage reward in World 23, which takes place in Bowser's Castle.

Dr. Bowser skill has him breathe fire[1] on random row(s) of the stage, clearing objects. [2] Viruses affected shown to be burnt. In Stage Mode, this affects two adjacent rows, while in Versus Mode, it affects one, two or three adjacent rows. The number of rows cleared in Versus Mode depends on Dr. Bowser's skill level. The row(s) affected by this skill are required to have at least one object that can be cleared, but unlike other doctors whose skills target randomly, viruses are not prioritized meaning that rows that do not have viruses are potentially targeted even though there are still viruses in the stage. In the case this skill clears two or three rows, if the topmost row(s) are empty or has unclearable objects and there are at least two rows of clearable objects next to each other, this skill might target the unclearable row(s) near the top alongside the clearable row(s) even if there are enough rows for this skill to clear.

Dr. Bowser reappears in Mario Kart Tour starting from the May 2022 Peach vs. Bowser Tour. Additionally, Dr. Bowser can be namedropped as an answer to a question concerning the name of Bowser's son during the Wonder Effect of Taily's Toxic Pond in Super Mario Bros. Wonder.

Rookie is Bowser's amnesiac alter ego and Popple's accomplice in Mario & Luigi: Superstar Saga and its remake, Mario & Luigi: Superstar Saga + Bowser's Minions. Bowser assumes this identity after being shot out of a cannon by Sgt. Starshade, (and subsequently hit on the head by Captain Goomba in the 3DS remake) Rookie's personality is drastically different than Bowser's; he is extremely trusting, loyal, and ultimately submissive to Popple, whereas his normal self is arrogant and believes others are beneath him. He memory is ultimately restored when he is flying through the air, clinging to the out-of-control Beanstar, but he loses his grip and is crash-lands in Stardust Fields.

A scanned page from "Beauty and the Beach" Ka-Hoopa Super Mario Bros. ("Beauty and the Beach") Ka-Hoopa is a persona King Koopa assumes in "Beauty and the Beach". He is in charge of an island populated by Mushrooms, and is using them to ignite the volcano using Bombs, which will reportedly scare away the various Trouters surrounding the island, although his actual plan is to transform them all into Fryguys via the volcano's eventual eruption. He makes no move to make trouble for Mario and company, and even discloses his plan, before going off on his way, telling them that the volcano will erupt in five minutes. His plan is ultimately foiled by Mario and Toadstool, who construct a huge pipe system out of hollow bamboo, which stifles the volcano. As Ka-Hoopa, he is clad in a purple Hawaiian shirt and a flower garland (which he then throws away because "[he] hates anything that's louder than [he is]"), and while in charge of the island, he enforces a dress code which decrees everyone to dress in beach attire as well.
