World 8 is a cosmic-themed world in Dr. Mario World. It is inspired by Super Mario Galaxy and Super Mario Galaxy 2, featuring elements from these games such as the Comet Observatory and planets from the Good Egg, Yoshi Star and Spin-Dig Galaxies. The first area is available as of 1 AM PT on Oct. 4[1], while the second area, challenge stages and special stage are available as of 5 PM PT on Oct. 17[2].

When the areas are cleared, Lumas of different colors appear, including yellow, blue, green, and red, along with Toads in astronaut suits, Star Bunnies and Octoombas. The viruses featured in the regular stages are red, green and light-blue.

Area 1

Image Stage Overview
Stage 281 from Dr. Mario World Stage 281 This stage introduces Keys, which float to the top of the stage until it collides with an object, and will not disappear until it comes into contact with a door. The objective of this stage is to unlock all doors. Hidden viruses are found in the third and fourth rows of Brick Blocks.
Stage 282 from Dr. Mario World Stage 282 The objective of this stage is to unlock all doors. This stage contains viruses that are blocked by crates (each requiring 4 hits to be destroyed) except for the top, with two keys outside of it.
Stage 283 from Dr. Mario World Stage 283 The objective of this stage is to eliminate all viruses and unlock all doors. This stage features cages that require clearing Lock Blocks to unlock and Floatie Viruses that obstruct the path between the keys and the doors, in addition to crates. Shells and exploders (added since version 1.3.5) are available to help clear the path.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 41 to 45.
  • Exploders are added directly above the keys, replacing crates.
  • The virus in the top-left is changed to a Floatie Virus while the virus directly below it, originally a Floatie Virus, is changed to a regular virus.
  • Two additional viruses are added directly above the bottom-most row of boxes.
  • The bottom-most row of crates now require two hits to be destroyed instead of three.
Stage 284 from Dr. Mario World Stage 284 The objective of this stage is to eliminate all viruses and collect all purple coins. The purple coins at the top and bottom rows must be collected before the Caged Coins at the left and right columns are unlocked.
Stage 285 from Dr. Mario World Stage 285 The objective of this stage is to eliminate all viruses and collect all pink coins from Brick Blocks. This stage contains viruses above and below Empty Blocks on the top part of the stage, in addition to shells and Exploders. Viruses in the shape of a "U" are on the bottom-left part of the stage, and on the bottom-right, Brick Blocks are found, with the middle hiding hidden viruses and the outer layer shaped like a "U" containing coins.
Stage 286 from Dr. Mario World Stage 286 This stage introduces colorful crates, which can only be broken if a like-colored match is made adjacent to them. The objective of this stage is to eliminate all viruses.
Stage 287 from Dr. Mario World Stage 287 The objective of this stage is to eliminate all viruses. This stage features Floatie Viruses surrounded by colorful crates, with a number of viruses surrounding the light-blue colorful crates.

The capsule amount is increased from 39 to 42 since version 1.3.5. Besides that, this stage is otherwise the same.

Stage 288 from Dr. Mario World Stage 288 The objective of this stage is to eliminate all viruses. This stage features Lock Blocks and Floatie Viruses situated between rows of cages.
Stage 289 from Dr. Mario World Stage 289 The objective of this stage is to eliminate all viruses and unlock all doors. This stage featues half-frozen viruses forming a square, along with crates, keys and locks forming a diamond with Brick Blocks inside the formation.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 39 to 43.
  • A number of crates changed from requiring three hits to two hits.
  • Three hidden viruses are found in Brick Blocks.
Stage 290 from Dr. Mario World Stage 290 The objective of this stage is to eliminate all viruses and collect all purple coins. This stage features caged purple coins locked behind cages, which requires collecting the purple coins in the middle and the Lock Block to be cleared to fully unlock. In addition, Caged Coins are found in Brick Blocks that are unlocked after the first set of caged purple coins are collected.
Stage 291 from Dr. Mario World Stage 291 The objective of this stage is to eliminate all viruses and destroy all crates. There are light-blue cages, with the corresponding Lock Block located in the middle of the crates. Ice Fans and bubble viruses are found in the bottom-right formation, in addition to the Lock Block. Shells can be found in the bottom 3 rows of cages.
Stage 292 from Dr. Mario World Stage 292 The objective of this stage is to eliminate all viruses. This stage contains crates and Brick Blocks locked behind red cages, with the Lock Block situated at the topmost row. It also features Floatie Viruses on the left and right columns, along with exploders confined within the structure.
Stage 293 from Dr. Mario World Stage 293 The objective of this stage is to collect all pink coins in Brick Blocks and purple coins. This stage features Brick Blocks containing pink coins, along with purple coins (both caged and uncaged) interspersed among viruses at the third to seventh rows. Additional viruses are found in the first two rows.

Prior to version 1.2.0, the stage layout was different. The following changes were made:

  • The top three purple coins are changed to Caged Coins. Collecting the existing coins will unlock the third row of Caged Coins, and collecting that row of coins will unlock the rest of the coins.
  • The top row of Brick Blocks do not contain pink coins.
  • Additional viruses are added to the third row.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

Stage 296 from Dr. Mario World Stage 296 The objective of this stage is to eliminate all viruses. The stage layout is shaped like a mirrored percentage symbol (%). On the "dot" portions, the Ice Fans are surrounded by viruses, plus there's a Lock Block located on the top portion. The Lock Block will unlock the exploders, which are sandwiched between fully frozen viruses.
Stage 297 from Dr. Mario World Stage 297 The objective of this stage is to eliminate all viruses and unlock all doors. Between the keys and doors are colorful crates. Viruses are found on the left and right of the colorful crates and doors, and the middle column is empty.
Stage 298 from Dr. Mario World Stage 298 The objective of this stage is to eliminate all viruses. This stage features viruses hugging against Ice Fans, along with colorful crates that block the other side of the viruses and a lone virus in the top-left corner. Shells are available in this stage.

This stage will scroll for one line each time enough objects are cleared, and will stop after 7 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Star Bunny assistant upon completing this stage.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 33 to 37.
  • Additional colorful crates are scattered throughout the beginning portion of the stage.

Area 2

Image Stage Overview
Stage 301 from Dr. Mario World Stage 301 This stage introduces the Capsule Plus, which when cleared, will increase the capsule amount by the number displayed. Every time a Capsule is added to the stage, the number for every Capsule Plus is reduced by one, disappearing entirely if the number reaches zero. The objective of this stage is to eliminate all viruses.

Stage 307 from Dr. Mario World Stage 307 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and collect all purple coins. This stage contains purple coins shaped like a "U", with viruses found within the boundary of coins. Every Brick Block contains pink coins that must be collected.
Stage 308 from Dr. Mario World Stage 308 The objective of this stage is to eliminate all viruses and collect all purple coins. This stage contains viruses arranged in a triangle that are blocked off by fans, colorful crates and purple coins. The Caged Coins requires the top-most purple coins to be collected before the next top-most purple coins will be unlocked, two at a time, with the two bottom-most coins being unlocked simultaneously when both remain.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

This stage will scroll for one line each time enough objects are cleared, and will stop after 7 lines are introduced.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Crates and Brick Blocks are removed from the bottom-most row.
  • The two bottom-most Capsule Plus capsules start with a number of 10 instead of 7
  • An exploder covered in dust replaces a dust virus at the middle of the second bottom-most row.

Stage 312 from Dr. Mario World Stage 312 The objective of this stage is to eliminate all viruses. This stage contains two groups of viruses each shaped like right-angle triangles, and among them three Floatie Viruses are found, two on one side and one on the other. Separating the two groups are crates and Ice Fans.
Stage 313 from Dr. Mario World Stage 313 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and collect all purple coins. This stage is shaped like a triangle, in which purple coins from the bottom row will need to be collected to unlock the Caged Coins found in the next-bottom row, and so on. Pink coins are found in all Brick Blocks. Viruses are found on the side of the triangle except the topmost row and the bottom-most row.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 42.
  • Two exploders are added in the second bottom-most row between Brick Blocks.
Stage 314 from Dr. Mario World Stage 314 The objective of this stage is to eliminate all viruses. This stage is shaped like a four-petaled flower, where viruses and Ice Fans form the petals, while the colorful crates form the pistil. An exploder can be found in the middle of the colorful crates.
Stage 315 from Dr. Mario World Stage 315 The objective of this stage is to eliminate all viruses. In this stage, Capsule Plus capsules are found in cages surrounded by crates, which is unlocked by clearing the Lock Block in the middle, also surrounded by crates. Shells can be found in the same rows as the Capsule Plus capsules. Viruses are found diagonally next to the shells and hugging the sides from the fourth to sixth rows.
Stage316 from Dr. Mario World Stage 316 The objective of this stage is to collect all pink coins from Brick Blocks and collect all purple coins. This stage contains three columns of purple coins in Bubbles, along with Brick Blocks containing pink coins on the leftmost and rightmost columns. Viruses are found in the other columns.

Stage 319 from Dr. Mario World Stage 319 In this stage, Capsule Plus capsules are scattered throughout Brick Blocks and Empty Blocks. Hidden viruses can be found in some of the Brick Blocks.
Stage 320 from Dr. Mario World Stage 320 The final stage is a scrolling stage that requires all viruses to be eliminated and all Brick Blocks to be destroyed. It starts out with a layer of viruses, followed by Capsule Plus capsules within colorful crates and crates. Following that are another group of viruses, and after that are colorful crates among Brick Blocks. Another group of viruses appear after that, and later on are exploders surrounded by Brick Blocks and crates. The topmost line contains more viruses. This stage will scroll for one line each time enough objects are cleared, and will stop after 11 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 9. Upon completion, players earn a free Staff Ticket that lets them unlock either a new doctor, a new assistant, or leveling up an existing doctor or assistant.

Challenge stages

Image Stage Overview
Stage 8A from Dr. Mario World Stage 8A Within an 85 second time limit, players need to eliminate all viruses. This stage consists of dust viruses on the two leftmost and two rightmost columns. Additionally, eight of each color of half-frozen viruses are found, with the green ones located on the corners, while the light-blue and pink ones are found in the middle column. Blocking the pink and light-blue viruses are light-blue and pink crates respectively.
Stage 8B from Dr. Mario World Stage 8B Within an 80 second time limit, players need to eliminate all viruses, destroy all Brick Blocks and collect all the Virus Coins. Floatie Viruses are found in this stage, and some Brick Blocks are half-frozen. There are only two colors (pink and light-blue) of viruses and capsules in this stage.
Stage 8C from Dr. Mario World Stage 8C Within a 85 second time limit, players need to eliminate all viruses and collect all pink coins from Brick Blocks. The viruses are patterned after a dollar symbol, with the viruses forming an "S" while the Brick Blocks form two vertical lines being crossed by the "S". The viruses require the yellow Lock Block to unlock the yellow cages. Players earn 1000 coins and a Staff Ticket (3 Hearts prior to version 1.2.0) if they complete this stage.

Prior to version 1.2.0, the virus icon showed only the yellow virus, which supposedly means that they are only needed to be eliminated. This is an oversight since there are 20 viruses altogether, and eliminating the non-yellow viruses count towards the total.

Special Stage

Image Stage Overview
World 8's Special Stage from Dr. Mario World Special Stage This objective of this stage is to eliminate all viruses and unlock all doors. There are three parts in this stage, and each part consists of viruses on each column obstructing the key from reaching the door. This stage will scroll for one line each time enough objects are cleared, and will stop after 12 lines are introduced. Completing this stage awards the player 1000 coins, and as of Version 1.2.0, Dr. Luma is also awarded.

  • Dr. Mario World Twitter:
    • Red: "Apparently new stages are gonna be added at 1 AM PT on Oct. 4. We might get to travel to outer space and see a whole new galaxy! Wait a minute…these round stars sure do look familiar..."[1]
    • Blue: "It appears that new stages have been added once again! The structure seated among them is known as the Comet Observatory, and it's used for travel from galaxy to galaxy. Such a shame it can't be used to escape these pesky doctors!"[2]
