World 28 is a cloud world with floating platforms resembling Bouncy Cloud Boomerangs from New Super Mario Bros. U in Dr. Mario World similar to World 15, except it takes place at night. The first area is available as of 11 PM PT on May. 6, 2021[1], while the second area, challenge stages and special stage are available as of 11 PM PT on May. 20, 2021[2].

When an area is cleared, Paratroopas, Parabombs, and Foos appear. The viruses featured in the regular stages are red, green and light-blue.

The objective of this stage is to eliminate all viruses (34) and collect all keys (3).

This stage introduces muddy coins, which are coins covered in mud that will require capsule matches next to them or direct elimination on them to remove the mud, which can require multiple matches to remove them. In this stage, there are small muddy coins in between the keys and doors, where the ones in the middle column each require two hits to clear, while the ones in columns #2 and #5 each require only one hit to clear. Viruses are in between the muddy coins for rows #5 to #8, floatie viruses are in between the muddy coins in #2, and ice is in between the muddy coins in rows #3 and #4. Some of the ice contain viruses and electric exploders.

The objective of this stage is to collect all muddy coins (4).

In this stage, each of the medium-sized muddy coins is surrounded by colorful crates, where they have different amounts of mud covering them. The middle contains a large flask. The filled flask in this stage will shoot out either colorful crates or exploder.

The objective of this stage is to eliminate all viruses (28).

In this stage, the objects form an "X" pattern composed of crates, colorful crates, bubbles containing viruses and floatie viruses, and a green lock block in the middle. The top contains a triangle of Empty Blocks, the sides contain cages where one cage on each side contains a lock block, and the bottom has cages below the crates and colorful crates.

The objective of this stage is to eliminate all viruses (30), collect all coins from Coin Blocks (24) and collect all muddy coins (8).

In this stage, there are two groups of 15 viruses and one exploder, and two columns of four Brick Blocks (all containing coins), where there is a group of 4 medium-sized muddy coins in between each of both groups. For each group of viruses, eight of them are half-frozen, arranged in a checkered pattern.

The objective of this stage is to eliminate all viruses (26) and collect all purple coins (32).

In this stage, purple coins (caged and uncaged) are arranged in rows of four, where they are in between colorful crates and rows of viruses. Some of the rows of viruses have gaps in between, and some of the viruses are encased in ice.

Purple coins are unlocked in the following sequence:

  1. The 8 coins in rows #2 and #3.
  2. The 8 coins in rows #4 and #5.
  3. The 8 coins in rows #6 and #7.
  4. The 8 coins (initially half-frozen) in rows #8 and #9.

The capsule amount is increased from 25 to 29 since June 3, 2021. Besides that, this stage is otherwise the same.

The objective of this stage is to eliminate all viruses (25), collect all coins from Coin Blocks (66) and break all barriers (7).

In this stage, barrels are arranged in an L-shape and are surrounded by Brick Blocks, of which only the middle one in row #2 and the second on in row #7 are empty while the rest are Coin Blocks. All the barrels contain exploders. Shells are found on the rightmost two spots in rows #6 to #8, and bubbles containing viruses are found in the rest of rows #2 to #5. The topmost row contains barriers and two ice cubes containing a pop cannon on the left and a virus on the right.

The objective of this stage is to eliminate all viruses (28).

In this stage, the middle three columns contain a ring of cages containing keys and doors, and within the ring are bubbles where all but two contain floatie viruses. On both the left and right sides, there are viruses in the formation of pins, where the bottom half has crates and the top half has a barrel containing lock blocks within. The red lock block is in the left barrel, while the green lock block is in the right barrel.

The objective of this stage is to collect all virus coins (17) and collect all muddy coins (20).

In this stage, Brick Blocks are arranged in diagonal lines, and in between the lines are Empty Blocks, small-sized muddy coins, exploders, and shells. Some of the exploders are half-frozen. All Brick Blocks contain hidden viruses, and some of those viruses have virus coins behind them. The filled flask in this stage will shoot out either viruses or exploders.

The objective of this stage is to collect all purple coins (16) and collect all muddy coins (16).

In this stage, dust clouds are separated into four quarters where they contain purple coins (caged and uncaged), keys, doors, and viruses. The two higher quarters are joined in the middle by a dust cloud containing a door. A fully frozen key is located directly below that dust cloud. Viruses are in between the two lower quarters, while a shell and Brick Blocks are in the row in between the groups of dust clouds. The two leftmost and two rightmost Brick Blocks contain doors.

Purple coins are unlocked in the following sequence:

  1. The 4 coins inside the lower-left group of dust clouds.
  2. The 4 coins inside the lower-right group of dust clouds.
  3. The 4 coins inside the upper-left group of dust clouds.
  4. The 4 coins inside the upper-right group of dust clouds.

The objective of this stage is to eliminate all viruses (76) and collect all keys (8). The only colors available in this stage are red and light-blue.

In this stage, the keys are in the leftmost and rightmost columns, and their corresponding doors are found in the last four revealed rows, where they are inside cages. At the beginning of the stage, there are colorful crates and viruses alongside the keys, where some of the viruses are half-frozen or inside bubbles. Muddy coins are found starting from the first-revealed row, and together with colorful crates, are found until the ninth-revealed row. Floatie viruses are found starting from the fifth-revealed row. The lock blocks are found in the eighth- and ninth-revealed rows. Brick Blocks (all containing hidden viruses) are found starting from the tenth-revealed row where exploders are also found. Some of the objects throughout the stage are encased in ice.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 13 lines are introduced.

The objective of this stage is to eliminate all viruses (32), collect all keys (12) and collect all muddy coins (8).

In this stage, there are four groups of objects surrounding a ring of muddy coins in the middle. The top-left and bottom-right groups have keys with crates in between them and the doors. The top-right and bottom-left groups contain viruses where some of them are half-frozen. In the same rows as the crates, each has a shell and a rotating cannon (one of them half-frozen).

The objective of this stage is to eliminate all viruses (32) and collect all purple coins (16).

In this stage, the uncaged purple coins form a right-angle line alongside Brick Blocks, while the caged coins form right-angle lines alongside Empty Blocks. Dust viruses are found in between the lines with caged coins. A column of half-frozen viruses is found in the rightmost column, while rows #8 and #9 contain shells, where the former has fully frozen viruses and the latter has half-frozen viruses and Brick Blocks.

Purple coins are unlocked in the following sequence:

  1. The 8 coins in both row #6 and column #8.
  2. The 5 coins in both row #4 and column #6.
  3. The 3 coins in both row #2 and the first coin in column #4.

The objective of this stage is to eliminate all viruses (19), collect all coins from Coin Blocks (39) and break all colorful crates (14).

In this stage, the top-right and bottom-left consist of Brick Blocks (all containing coins), and in between them, there are diagonal lines of viruses covered in clouds, with lines of crates and/or colorful crates in between them.

The objective of this stage is to eliminate all viruses (24), break all crates (5) and collect all keys (12).

In this stage, the middle column contains crates that each take four hits to fully destroy, while the other odd-numbered columns contain keys and doors encased in ice cubes. The fourth and sixth columns contain barrels containing exploders, while the second and eighth columns contain only bubbles. The middle five columns also contain viruses.

The objective of this stage is to eliminate all viruses (10), collect all coins from Coin Blocks (18) and collect all keys (5).

In this stage, bubbles containing floatie viruses are in the odd-numbered columns, where all but the ones in the leftmost column are surrounded by colorful crates. The two barrels directly above the bubbles contain keys and doors, while the rest of the barrels contain exploders. Of the Brick Blocks, only the second one in the eighth column doesn't contain anything, while the rest are Coin Blocks.

The objective of this stage is to eliminate all viruses (20) and collect all keys (2).

In this stage, the middle contains muddy coins in a checkered pattern, where the gaps are filled with viruses, lock blocks, cages, and Brick Blocks. The Brick Blocks contain hidden viruses. They are surrounded by ice cubes and one shell. The keys and doors are in the leftmost and rightmost columns.

Prior to June 3, 2021, the stage layout was different. The following changes were made:

  • The light-blue viruses are now inside light-blue cages (the light-blue lock block is still there even before the change).

The objective of this stage is to eliminate all viruses (27) and collect all coins from Coin Blocks (45).

In this stage, the Brick Blocks and crates are arranged in rows in the odd-numbered rows up to row #9 with a few Brick Blocks found in between the rows, and barrels containing viruses are found in the even-numbered rows between those rows. The Brick Blocks in rows #1, #5, #6, #8 and #9 are Coin Blocks, while the rest do not contain anything.

The objective of this stage is to eliminate all viruses (21) and collect all virus coins (46).

In this stage, barriers are found around the upper portion, where fully-frozen viruses and a half-frozen shell are also found. The topmost row of barriers is also half-frozen. Virus coins are found behind the group of objects except for the last barrier in the third row. Below the group of barriers, the even-numbered columns contain viruses below pop cannons, where each column has two floatie viruses. Crates are found in the odd-numbered rows.

The objective of this stage is to eliminate all viruses (24), collect all coins from Coin Blocks (24) and break all crates (26).

In this stage, a group of viruses is found in the middle five columns below a group of crates. Ice cubes are found in the odd-numbered columns among the crates, except the middle column which contains an exploder. The Brick Blocks (all containing coins) are found in the second and eighth columns, where viruses are in between them.

The last stage of the first area, the objective of this stage is to eliminate all viruses (111) and collect all muddy coins (41).

At the beginning of the stage, the bottom half contains rows containing viruses and muddy coins, while the top half contains barrels (containing viruses, exploders, and a shell) and muddy coins. In the row of Brick Blocks in the fifth row, the second block contains a shell, the last block contains an exploder, while the rest contain viruses. In the row of Brick Blocks in the first row, the first six blocks contain viruses and the eighth block contains an exploder, while the rest are empty. In each of the first six revealed rows, there is one muddy coin in between fully-frozen viruses, where only the one in the sixth row has three different colors in the same row. The seventh-revealed row contains barrels containing viruses, and the eighth-revealed row contains a shell in a barrel and a row of muddy coins. The ninth-revealed row contains dust clouds and a half-frozen virus, where the dust clouds contain viruses, an exploder and a couple of ice fans. The last-revealed row contains half-frozen viruses in between two muddy coins.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 10 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Parabomb assistant upon completing this stage.

The objective of this stage is to eliminate all viruses (30), collect all keys (3) and collect all muddy coins (14).

In this stage, muddy coins of all sizes are scattered throughout the stage, and half-frozen objects mostly consisting of viruses (the others being an exploder and a pop cannon) are in between most of the gaps. The ninth row is the only row where the viruses are not encased in ice. The three middle columns contain the keys and doors, and two dust viruses are below each of the doors.

The objective of this stage is to eliminate all viruses (30) and collect all purple coins (12).

In this stage, the bottom-left and bottom-right groups contain exploders surrounded by caged viruses, with their corresponding lock blocks inside bubbles at the top corners. The middle columns contain viruses surrounded by cages, most of them containing Brick Blocks, and the corresponding lock block is in the middle column below the cages. Between the middle ring of cages, there are columns of fully frozen viruses and pop cannons. Columns of purple coins (caged and uncaged) are next to the columns of fully frozen objects.

Purple coins are unlocked in the following sequence:

  1. The 2 coins in row #6.
  2. The 2 coins in row #5.
  3. The 2 coins in row #4.

The objective of this stage is to eliminate all viruses (40), collect all coins from Coin Blocks (48) and collect all keys (1).

In this stage, the two leftmost and two rightmost columns contain rows of two viruses and Brick Blocks, while the middle five columns contain Brick Blocks in rows of three, where viruses are exploders are above the ones not in the middle column. Of the Brick Blocks, columns #3, #4, #6 and #7 are Coin Blocks, one of the blocks in each row of two of Brick Blocks contain Capsule Plus, while the rest contain hidden viruses.

The objective of this stage is to eliminate all viruses (19) and collect all virus coins (1).

In this stage, colored cages are formed in a checkered pattern, where bubbles are within the gaps except for the lock block in the bottom-left corner. The other two lock blocks are in the top corners inside bubbles. The bubbles that are filled contain viruses, exploders and electric exploders. The single virus coin is in the middle of the stage, where it is covered by all three colors of cages.

The objective of this stage is to eliminate all viruses (24), collect all purple coins (23) and collect all keys (8).

In this stage, three diagonal lines containing keys, dust clouds, and doors cut across two halves that each contain purple coins (caged and uncaged), exploders, frozen viruses, and frozen electric exploders. The exploder in the upper-left half is half-frozen. The upper-left half also contains Brick Blocks. The dust clouds all contain rotating cannons.

Purple coins are unlocked in the following sequence:

  1. The 14 coins in the odd-numbered columns.
  2. The 9 coins in the even-numbered columns.

The objective of this stage is to eliminate all viruses (55) and collect all virus coins (19).

In this stage, there are five groups of cages: three of them are in 3x3 squares in between rows that contain Empty Blocks and Brick Blocks, while the other two are in L-shapes. The top-left and top-right corners contain Brick Blocks surrounded by exploders, where each group of blocks contains a lock block inside and viruses in the rest. Large virus coins are behind each of the group of cages and the Brick Blocks surrounding the exploders, while the small virus coins are behind the caged viruses in L-shapes. A green lock block is in the middle column in between the Empty Blocks.

The objective of this stage is to eliminate all viruses (12), collect all coins from Coin Blocks (45) and collect all muddy coins (4).

In this stage, Brick Blocks (all containing coins) are in groups of three: four of them near the corners and one of them in the middle. Muddy coins are in between the groups of blocks in the corners, while exploders are found in the gaps in between the blocks and muddy coins, where the ones near to the corners are half-frozen. Rows or columns of three floatie viruses are found near the groups of blocks near the corners.

The objective of this stage is to eliminate all viruses (18), collect all purple coins (20) and collect all keys (8).

In this stage, the barrels are arranged in an eight-pointed star, where each point consists of three barrels. The barrels mostly contain purple coins (caged and uncaged), with the remaining ones containing keys and doors. Rows of keys and doors in cages are found around each of the corners, where half-frozen viruses are also found. The lock blocks are also half-frozen and are near the middle-left and middle-right sides.

Purple coins are unlocked in the following sequence:

  1. The 3 coins in the bottom three barrels in the middle column.
  2. The 3 coins on the leftmost barrels in each of rows #6 to #8.
  3. The 2 coins in the second and third barrels in row #5.
  4. The 2 coins on the leftmost barrels in each of rows #2 and #3.
  5. The 3 coins in the top three barrels in the middle column.
  6. The 3 coins on the rightmost barrels in each of rows #2 to #4.
  7. The 2 coins in the fourth and fifth barrels in row #5.
  8. The 2 coins on the rightmost barrels in each of rows #7 and #8.

The objective of this stage is to eliminate all viruses (16) and collect all virus coins (13).

In this stage, groups of ice are separated by lines of colorful crates, where a crate is in the middle, rotating cannons are in the corners and Empty Blocks are in the sides. In each group of ice, there is one handy bottle and four floatie viruses. The handy bottles are labeled with different objects, where the top-left one contains skill chargers, the top-right one contains pop cannon, the bottom-left one contains shells and the bottom-right one contains exploders.

The objective of this stage is to eliminate all viruses (38), collect all keys (3) and collect all muddy coins (21).

In this stage, muddy coins are scattered, and the gaps are filled with objects, mostly viruses. Dust clouds are found at the beginning of the stage, where only the leftmost dust cloud doesn't contain anything while the rest contains viruses. Medium-sized muddy coins are found at the beginning of the stage, while muddy coins of all sizes are found starting from the second-revealed row. The keys are in the first-revealed row and the doors are in the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

The objective of this stage is to collect all virus coins (9) and collect all keys (4). The only colors available in this stage are green and light-blue.

In this stage, there are four columns of dust clouds that do not contain anything in the even-numbered rows surrounded by barrels and ice that contain exploders and electric exploders. Among the barrels, the ones in the topmost row contain doors, the ones in the ninth row contain keys while the rest are empty.

The objective of this stage is to eliminate all viruses (18) and collect all coins from Coin Blocks (36).

In this stage, the four leftmost columns contain rows of cages, half-frozen exploders, and fully-frozen Brick Blocks (all containing coins). The rightmost column contains a pop cannon that targets shells and half-frozen lock blocks. The sixth to eighth columns contain rows of fully-frozen viruses and bubble machines. The middle column contains crates in the same row as the half-frozen exploders.

The objective of this stage is to eliminate all viruses (32), break all barriers (14) and collect all purple coins (21). The only colors available in this stage are red and light-blue.

In this stage, the rows of Brick Blocks are separated by caged coins in the middle column. Within each row of Brick Blocks on both sides, each of them has one purple coin, one barrier, and two viruses. All the barriers start out inactive after they are freed from the blocks. Purple coins are unlocked in the following sequence:

  1. The 14 coins inside the Brick Blocks.
  2. The 7 coins in the middle column.

The objective of this stage is to eliminate all viruses (16), collect all keys (8) and collect all muddy coins (16).

In this stage, the keys are found in the third and ninth rows, while the doors are found two rows above the keys. The keys and doors close to the middle in rows #3 and #7 are surrounded by muddy coins and fully frozen viruses. Electric exploders are found in the leftmost, middle, and rightmost columns, while half-frozen lock blocks are found in row #5. The electric exploders in the middle column are in between Empty Blocks.

The objective of this stage is to eliminate all viruses (36) and collect all coins from Coin Blocks (63).

In this stage, columns of three crates and Brick Blocks (all containing coins) are arranged in a checkered pattern, and in between each of the gaps are a colorful crate and two half-frozen viruses except for the top-left corner. The tenth row consists of pop cannons in the even-numbered columns and a shell at the leftmost part of the row.

The objective of this stage is to eliminate all viruses (28) and collect all purple coins (26).

In this stage, purple coins are found in L-shapes rotated 90 degrees clockwise, where there are L-shapes rotated 90 degrees clockwise of Brick Blocks and crates intertwined with half-frozen viruses. All the Brick Blocks contain hidden viruses.

The objective of this stage is to eliminate all viruses (27), collect all coins from Coin Blocks (24) and collect all muddy coins (5).

In this stage, the Brick Blocks (all containing coins) are found in rows #4 and #5, the medium-sized muddy coins are found in the two topmost rows and the large-sized muddy coins are found in rows #6 to #8. Viruses and half-frozen viruses are found next to the large-sized muddy coins, while viruses and bubbles (containing exploders and viruses) are found near the medium-sized muddy coins.

The objective of this stage is to collect all virus coins (27) and collect all purple coins (15).

In this stage, each virus coin that covers a 2x2 area is covered by a caged coin and a floatie virus, and they are segmented by crates, colorful crates, and cages. Those cages have virus coins behind them. The gaps in between the crates, colorful crates, and cages are filled with purple coins and lock blocks.

Purple coins are unlocked in the following sequence:

  1. The 6 coins in rows #1, #4 and #7.
  2. The 9 coins in rows #3, #6 and #9.

The objective of this stage is to eliminate all viruses (16), collect all coins from Coin Blocks (48) and collect all keys (8).

In this stage, rows of six Brick Blocks are located below rows of barrels. The leftmost and rightmost blocks of each row contain keys, while the rest contain coins. The leftmost and rightmost barrels contain doors, while the rest contain viruses and electric exploders. Additionally, colorful crates and floatie viruses are found next to the rows of blocks and barrels.

In the final stage of this world, the objective is to eliminate all light-blue viruses (22) and collect all muddy coins (20). The only colors available in this stage are red and light-blue.

At the beginning of this stage and the first-revealed row, in addition to several small muddy coins and three medium-sized muddy coins, there are crates arranged in columns of two or four, ans well as light-blue viruses in rows and columns of two or three. Additionally, the red viruses are all fully frozen. The second-revealed row consists of floatie viruses where the light-blue ones are inside bubbles. The third- and fourth-revealed rows contain three more medium-sized muddy coins, while the fifth- to seventh-revealed rows contain the only large-sized muddy coin. More crates are found in the fourth- to sixth-revealed rows, as well as the last-revealed row. The sixth- and eighth-revealed rows contain light-blue floatie viruses inside bubbles, while the seventh-revealed row contains fully-frozen red viruses. A shell is found in the middle of the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 29. Upon completion, players earn a free Staff Ticket.

In this stage, the viruses and muddy coins are intersected within a checkered pattern, where crates and Empty Blocks are within the gaps. The ninth row consists of dust clouds (all containing nothing) and an exploder.

Completing this stage awards the player 500 coins.

Within a 50 second time limit, players need to eliminate all viruses (23), collect all coins from Coin Blocks (33) and collect all muddy coins (8). The only colors available in this stage are green and pink.

In this stage, the Brick Blocks (all containing coins) are arranged in the shape of a smiley face, where medium-sized and large-sized muddy coins are also found. pop cannons and exploders are within the gaps in row #4, while viruses are found within the other gaps within the first nine rows.

Completing this stage awards the player 500 coins.

In this stage, the keys and doors are scattered among bubbles, where some of them contain viruses and electric exploders. Only row #7 has viruses and electric exploders that are not inside bubbles. Empty Blocks are found in the second to fifth-revealed rows, and a bubble containing a shell is found in the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and a Staff Ticket.

The objective of this stage is to eliminate all viruses (38), break all the ice (44) and collect all keys (8).

In this stage, the middle consists of a diamond (viewable after the stage is fully scrolled) mostly by ice cubes, with exploders found at the top, middle, and bottom. Bubbles containing viruses are arranged in two triangular groups at the top-left and the bottom-right, while the rest of those rows are filled with normal viruses. The keys and doors are in all columns except for the middle column.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 3 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Lemmy.
