World 20 is a world taking place in a spooky forest in Dr. Mario World. The first area is available as of 12 AM PT on Sept. 24, 2020[1], while the second area, challenge stages and special stage are available as of 11 PM PT on Oct. 7, 2020[2].

When an area is cleared, Broozers, Boos, Peepas and Swoops appear. The viruses featured in the regular stages are red, green and light-blue.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (29) and collect all keys (8).

This stage introduces electric exploders, which will send out electric shocks to three nearby objects when matched or destroyed. In this stage, there are three rows of viruses with electric exploders in the middle, where two of them are inside barrels. Cages are found between the keys and doors in the second, fourth and sixth rows, with their corresponding lock blocks in the barrels in the topmost row inside barrels where viruses inside barrels are also found.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (21) and collect all coins from Coin Blocks (30).

In this stage, four rows containing Brick Blocks and Empty Blocks are lined up in the odd-numbered rows within the first seven rows, and below each of those rows, there are floatie viruses. The Brick Blocks in the odd-numbered columns are Coin Blocks, while the others contain hidden electric exploders.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (56).

In this stage, there are nine sections group in three-by-threes. The groups in the corners each contain nine viruses with five of them being hidden in clouds, the groups at the sides each contain an electric exploder in the middle of Brick Blocks (containing hidden viruses), crates and ice fans, while the middle group contains four clouded viruses with a shell in the middle.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (29) and collect all purple coins (32).

In this stage, there are barrels where they all contain purple coins (caged and uncaged). Bubbles containing viruses are in the second to seventh rows, while the eighth row has regular viruses.

Purple coins are unlocked in the following sequence:

  1. The 14 coins within row #2 to row #4, initially within the partially-damaged barrels.
  2. The 18 coins within row #5 to row #8, initially within the undamaged barrels.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (25) and collect all keys (2).

In this stage, the keys and doors are in the second and eighth columns, where the doors are behind cages and their corresponding lock blocks between the keys and doors inside bubbles. Electric exploders are also inside bubbles between the keys and doors. The Brick Blocks and crates are arranged in a checkered pattern, where the Brick Blocks all contain hidden viruses.

Starting with 38 capsules, the objective of this stage is to collect all coins from Coin Blocks (36) and collect all virus coins (5).

In this stage, the cages containing Brick Blocks and crates are formed in a pattern that spells "2=2", where viruses surrounding barrels containing lock blocks are within the gaps in each character. All the Brick Blocks in this stage are Coin Blocks, where large virus coins are located above and below them. The filled flasks in this stage will shoot out either floatie viruses or exploders.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (14) and destroy all Brick Blocks (27).

In this stage, two sets of Brick Blocks covered in ice cubes in the form of wavy lines are found, where between them are Brick Blocks, viruses and bubble machines. An exploder handy bottle is located at the left of the ninth row.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (15), collect all purple coins (14), and collect all keys (8).

In this stage, the keys and doors are in every column except for the middle column, where the keys are fully-frozen and the doors are half-frozen. There is nothing between the keys and doors in the four leftmost columns, but within the five rightmost columns, there are floatie viruses checkered among fully-frozen purple coins. On the eighth row, there are four crates on the left and four pop cannons on the right. A shell handy bottle is located in the middle of the seventh row.

Starting with 37 capsules, the objective of this stage is to eliminate all viruses (30) and collect all keys (9).

In this stage, there are three pop cannons each in between crates. Each pop cannon is aimed towards half-frozen viruses and exploder. Of the undamaged barrels, the two middle barrels in the topmost row contain viruses while the rest contain keys and doors. The partially-damaged barrels contain colorful crates and flasks. Most of the Brick Blocks contain hidden viruses except for the middle one in the topmost row, which contain a hidden door. The filled flasks in this stage will shoot out colorful crates.

Starting with 36 capsules, the objective of this stage is to eliminate all viruses (71) and collect all virus coins (52). The only colors available in this stage are red and light-blue.

This stage only contains red and light-blue objects. At the beginning of this stage, there is a row of viruses with a gap in the middle below a row of Brick Blocks, which contain four electric exploders. The initial four rows in the beginning and the next seven revealed rows contain viruses checkered among bubbles containing colorful crates and electric exploders each with a virus coin behind them. The viruses from the sixth-revealed row to the tenth-revealed row are half-frozen. Within the eighth-revealed row to the tenth-revealed row, there are three virus coins behind crates, half-frozen viruses and exploders. The final-revealed row contains a row of Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 11 lines are introduced.

Starting with 37 capsules, the objective of this stage is to collect all coins from Coin Blocks (24) and collect all virus coins (4).

In this stage, there are four rings of purple coins (caged and uncaged) each with an exploder inside and covering a large virus coin. The fifth row contains Brick Blocks that all contain coins, while the middle column contains bubbles with floatie viruses inside.

Purple coins are unlocked in the following sequence:

  1. The coins in columns #4 and #6, for a total of 16 coins.
  2. The rest of the coins in the stage, for a total of 32 coins.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (12) and collect all keys (8).

In this stage, there are two rings of fully-frozen exploders and barriers between the keys and the doors. Pop cannons are located in the middle column, the first two columns and the last two columns. Fully-frozen viruses are located in the first three rows in a checkered pattern with barriers in between.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (26) and collect all coins from Coin Blocks (36).

In this stage, floatie viruses are surrounded by crates, and surrounding it are Brick Blocks in a checkered pattern. Between the Brick Blocks, there are pop cannons and bubbles containing viruses. The 12 Brick Blocks nearest to the crates are Coin Blocks.

Starting with 40 capsules, the objective of this stage is to collect all virus coins (29).

In this stage, frozen viruses are grouped in fours, with electric exploders (in and out of barrels) placed around the sides and are all covering virus coins. The virus coin in the middle of the stage is covered by electric exploders (in and out of barrels), exploders and crates.

Starting with 31 capsules, the objective of this stage is to eliminate all viruses (28) and collect all coins from Coin Blocks (36).

In this stage, floatie viruses are placed in columns #3, #5 and #7 with pop cannons below them. The barrels contain viruses, while all the Brick Blocks are Coin Blocks. Within the same rows as the Brick Blocks are fully-frozen crates.

Starting with 34 capsules, the objective of this stage is to eliminate all viruses (28) and collect all purple coins (30).

In this stage, the crates separated viruses and dust viruses into two groups, where they are arranged in a checkered pattern. Purple coins (caged and uncaged) are found in the first eight rows. The lock blocks are in the first row in between fully-frozen viruses and in the middle of the third row, with the corresponding cages in the second row and above the green lock block. There is a shell and an exploder behind the green cages, while the other cages contain purple coins.

Purple coins are unlocked in the following sequence:

  1. The coins within rows #4 to #8, for a total of 16 coins.
  2. The 4 coins in row #3.
  3. The 2 coins in row #1.
  4. The 8 coins in row #2.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (21) and collect all virus coins (30).

In this stage, there is a border of Brick Blocks and purple coins surrounding eight viruses in the middle. All the Brick Blocks contain floatie viruses. Within rows #1 and #8, there are bubbles each containing a caged coin, with a bubble containing a shell in the top-right corner.

Purple coins are unlocked in the following sequence:

  1. The coins that are part of the border with the Brick Blocks, for a total of 13 coins.
  2. The coins inside bubbles starting from rows #1 and #8, for a total of 17 coins.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (40) and destroy all colorful crates (11).

In this stage, there are three groups of cages each with different colors, with their respective lock blocks located around the bottom-right of each group. The cages contain colorful crates, electric exploders, viruses and ice fans. Surrounding the cages are viruses.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (18), collect all coins from Coin Blocks (36), and collect all purple coins (17).

In this stage, the objects are arranged in a way that resembles two triangles stacked on top of each other by the tips. The triangles are formed by purple coins (caged and uncaged), Brick Blocks all containing coins, viruses, dust viruses and electric exploders.

Purple coins are unlocked in the following sequence:

  1. The 5 coins in row #9.
  2. The 3 coins in row #7.
  3. The 1 coins in row #5.
  4. The 3 coins in row #3.
  5. The 5 coins in row #1.

The last stage of the first area and starting with 45 capsules, the objective of this stage is to eliminate all viruses (52) and collect all keys (8).

In the beginning of this stage and the next two revealed rows, there are barrels at the three leftmost columns which contain exploders in the same row as the shells inside barrels, a lock block in one of the barrels in the second-revealed row and viruses in the others. Colorful crates are placed in the same row as the shells inside barrels, as well as below some viruses. All the keys are found in two spaces above, with the leftmost and rightmost keys (the latter visible after the stage scrolls) having their doors in the eighth-revealed rows.

In the third-revealed row, there are two pop cannon handy bottles. The bottles are caged along with the crates in the rest of the stage. The rest of the stage also has viruses, half-frozen floatie viruses and Brick Blocks, with the exception of the last revealed row, which is a row of Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Broozer assistant upon completing this stage.

The capsule amount is increased from 36 to 45 since June 2021. Besides that, this stage is otherwise the same.

In this stage, there is a row of floatie viruses below a row of crates and above a row of electric exploders. Above the row of crates is a row of 7 floatie viruses, where they are below crates arranged in a checkered pattern.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (32) and collect all keys (16).

In this stage, there are two groups of dust clouds surrounded by barrels on the left and right sides, with viruses and two lock blocks in between those barrels. Of the viruses, the viruses in the second and ninth rows are caged. The bottom-most barrels in the leftmost and rightmost columns contain exploders and the bottom-most barrels in the second and eighth columns contain pop cannons, while the other barrels contain keys and doors. Of the dust clouds, the three leftmost ones in the sixth row and the three rightmost ones contain exploders and shells, the bottom-most ones in the two leftmost and two rightmost columns contain doors, while the rest are dust viruses.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (28) and destroy all colorful crates (26).

In this stage, there are two groups of colorful crates in a checkered pattern, in which each of them have two rows of bubbles containing viruses and one row with only viruses. In between the two groups are two rows containing Brick Blocks and Capsule Plus each starting from +5. The Brick Blocks contain hidden Capsule Plus each starting from +5 and ice fans.

In this stage, each of the 6 virus coins is covered by clouded floatie viruses and crates. This stage contains a shell handy bottle and a flask in the topmost row. The filled flask in this stage will shoot out Brick Blocks.

In this stage, the purple coins are surrounded by red cages, and beside the red cages are light-blue cages. The cages contain viruses, keys, doors and crates. The lock blocks for those cages are located in the topmost row each in between viruses.

Starting with 35 capsules, the objective of this stage is to collect all coins from Coin Blocks (36) and collect all virus coins (21).

The virus coins are found in the first five rows, where the floatie viruses are each covering one while in between the rows of floatie viruses are Brick Blocks covering three virus coins.

In this stage, colorful crates are found throughout the stage, with the ones in rows #8 and #9 inside bubbles. Floatie viruses are found in rows #3 and #7, while purple coins (caged and uncaged) are located in rows #1, #3 and #7.

Starting with 45 capsules, the objective of this stage is to eliminate all viruses (64) and collect all keys (36). The only colors available in this stage are red and light-blue.

In this stage, the pairs of keys and doors are either half-frozen or have a virus in between them, or both. As the stage scrolls, there are a few crates found, and near the topmost portion of the stage, there are colorful crates with most of them being frozen.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 13 lines are introduced.

The capsule amount is increased from 39 to 45 since June 2021. Besides that, this stage is otherwise the same.

Starting with 32 capsules, the objective of this stage is to collect all virus coins (24).

In this stage, columns #3 and #7 have caged exploders and electric exploders. In between those columns are viruses, shells, exploders and colorful crates, with the topmost row containing the lock block in between crates. On the outside of the columns of cages are viruses grouped in rows of two, where they are either floatie viruses or viruses inside bubbles. The virus coins are behind the viruses in the two leftmost and two rightmost columns, as well as the viruses and shells in rows #2 to #8 in between the columns of cages.

In this stage, the objects are arranged in a pyramid, where purple coins (caged and uncaged) are found in the odd-numbered columns while crates and Brick Blocks (all containing coins) are in the even-numbered columns. Viruses are found throughout the structure, where many of them along with the caged coins are half-frozen.

Purple coins are unlocked in the following sequence:

  1. The 1 coin in row #9.
  2. The 2 coins in row #7.
  3. The 2 coins in row #5.
  4. The 2 coins in row #3.
  5. The 2 coins in row #1.

In this stage, purple coins are scattered throughout the stage, and alongside caged floatie viruses, their corresponding lock block and colorful crates, cover all the virus coins. A row of exploders is within the eighth row that is near the first uncaged purple coin and the lock block.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (29).

In this stage, there are cages where the red cages have bubble machines and Brick Blocks behind them while the green cages contain electric exploders. In the middle of each of the four groups of bubble machines is an exploder with viruses (two normal and two half-frozen surrounding it). The red lock block is in the middle of the Brick Blocks in the middle column while the green lock block is hidden inside the middle Brick Block in row #2. The other Brick Blocks within the middle three columns are hidden viruses. There are two exploder handy bottles in row #5 on the opposite ends.

Starting with 40 capsules, the objective of this stage is to collect all coins from Coin Blocks (36) and destroy all barriers (24).

In this stage, there are four groups of Brick Blocks with barriers inside them in the shape of triangles, with a Skill charger handy bottle in between all of them. The Brick Blocks in the even-numbered columns and the even-numbered rows (12 altogether) are Coin Blocks, while the rest of the Brick Blocks contain hidden viruses.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (34) and collect all keys (12).

In this stage, the middle column contains shells and caged electric exploders. In between the keys and doors are half-frozen viruses and colorful crates. The caged electric exploders' corresponding lock blocks are also caged alongside crates and a few viruses, with the corresponding lock block on the left side of row #9.

Starting with 20 capsules, the objective of this stage is to eliminate all viruses (15), collect all coins from Coin Blocks (18), and destroy all crates (18).

In this stage, the objects are arranged in three overlapping squares where the borders are crates, Brick Blocks and Empty Blocks, and within the gaps are viruses, shells and exploders. The Brick Blocks that contain coins are both of them in row #8, the first and last ones in row #5, the last one in row #3 and the first one in row #1. The Brick Blocks that contain Capsule Plus are in the first one in row #7 (+5), the second one in row #6 (+3), and the first one in row #2 (+7). The rest of the Brick Blocks contain hidden viruses.

In the final stage of this world and starting with 38 capsules, the objective is to collect all purple coins (41) and collect all keys (4).

At the beginning of the stage, purple coins are arranged in a checkered pattern with barrels containing viruses, flasks and electric exploders within the gaps. Both rows of barrels contain viruses. Keys are within the barrels in the first-revealed row, with their corresponding doors are within the final-revealed row, where they are also within barrels.

Beyond the row of barreled keys, some rows contain half-frozen flasks that will not activate as long as they contain ice. The second-revealed row contains electric exploders, the fifth-revealed row contains brick Blocks containing hidden viruses and electric exploders, and the eighth-revealed row contains Brick Blocks containing hidden viruses and purple coins. In between the aforementioned rows are viruses and fully-frozen purple coins, with additional fully-frozen purple coins in the last two revealed rows. The filled flasks in this stage will shoot out viruses.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 21. Upon completion, players earn a free Staff Ticket.

Within a 40 second time limit, the objective of this stage is to eliminate all viruses (39). The only color available in this stage is pink.

In this stage, pink is the only color found. Besides the viruses throughout the stage, there are barrels, crates and electric exploders. The undamaged barrels contain viruses and electric exploders, while the partially-damaged barrels contain only viruses.

Completing this stage awards the player 500 coins.

Within a 50 second time limit, the objective of this stage is to collect all purple coins (34). The only colors available in this stage are light-blue and pink.

In this stage, pink and light-blue are the only colors found. Purple coins (both caged and uncaged) are also found in five of the rows. The Brick Blocks in the eighth row contain hidden viruses. The barrels in the fourth and sixth rows contain floatie viruses and exploders, while the barrels in the second row contain caged coins. Other than that, the stage also has viruses in the first row and middle column, as well as exploders in the fourth and sixth rows.

Purple coins are unlocked in the following sequence:

  1. The 5 coins in row #9.
  2. The 5 coins in row #7.
  3. The 8 coins in row #5.
  4. The 8 coins in row #3.
  5. The 8 coins in row #2, initially inside barrels.

Completing this stage awards the player 500 coins.

Within a 57 second time limit, the objective of this stage is to eliminate all viruses (22) and destroy all colorful crates (36). The only colors available in this stage are green and pink.

In this stage, colorful crates are found within the odd-numbered columns within the sixth to ninth rows and the first and third rows, with the ones in the first row being half-frozen. Between the columns of colorful crates are half-frozen viruses, barriers and half-frozen pop cannons. In the fifth row, there are barriers with a fully-frozen shell in the middle. The second and fourth rows contain frozen viruses, while fully-frozen shells are in the middle of the first and third rows between the colorful crates.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (46).

In this stage, there are two groups of dust clouds. The lower group of dust clouds contains 2 floatie viruses, 3 ice fans and viruses in the others, while the higher group of dust clouds contain 2 electric exploders and viruses in the others. In the middle of the stage, there are 9 electric exploders covered in ice cubes. To the left and right of the group of electric exploders are viruses placed among crates, and each side has 3 floatie viruses. More ice fans are found on the group to the left.

Completing this stage awards the player 1000 coins and Dr. King Boo.
