World 13 is an underground crystal cavern world resembling Tilted Tunnel from New Super Mario Bros. U in Dr. Mario World. The first area is available as of 1 AM PT on Mar. 5, 2020[1], while the second area, challenge stages and special stage are available as of 2 AM PT on Mar. 19, 2020[2].

When an area is cleared, Bony Beetles, Bone Goombas, and Swoops appear. The viruses featured in the regular stages are red, yellow and blue, with the exception of one stage, which has green viruses in addition to blue.

In this stage, caged purple coins surround barrels, with some partially revealing its contents that include exploders and viruses. All but one unbroken barrel contain viruses, in which a purple coins if hidden there instead. The filled flasks in this stage will shoot out either shells or viruses.

Purple coins are unlocked in the following sequence:

  1. The coin in the middle, undamaged barrel.
  2. The 20 coins surrounding the barrels.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The available colors are changed to red and blue only.
  • Shells are added in the leftmost and rightmost of row #5.
  • All flasks are removed from this stage. Those filled flasks would have shoot out either shells or viruses.
  • All revealed viruses are changed to become half-frozen.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 34 to 33.
  • The first and last Brick Blocks in row #8 are replaced with pop cannons.
  • The objects between the leftmost and rightmost Empty Block in the topmost row are changed from 6 ice fans with an Empty Block in between them to 5 fully-frozen Brick Blocks in the middle. As a result of having more Brick Blocks than the objective requires, not all of them need to be broken.

The last stage of the first area and starting with 36 capsules, the objective of this stage is to eliminate all viruses (17) and collect all purple coins (12).

In this stage, all colorful crates are wrapped in bubbles, with several of them floated upwards toward viruses. The initial three rows and the three rows after that also contain half-frozen coins and dust viruses. Brick Blocks do not contain anything.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Purple coins are unlocked in the following sequence:

  1. The coin in row #3.
  2. The coin in row #2.
  3. The coin in row #1.
  4. The coins in the next three revealed rows, each revealed row containing one coin for a total of 3 coins.
  5. The 6 coins in the sixth-revealed row.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Bony Beetle assistant upon completing this stage.

Flasks are hidden in the Brick Blocks in the leftmost and rightmost columns in rows #5 and #9, and an exploder is hidden in a Brick Block in row #4 of the middle column. Viruses and floatie viruses are hidden in the other Brick Blocks. The filled flasks in this stage will shoot out either empty Brick Blocks or Brick Blocks with hidden viruses.

In row #1, barriers are above the bubble machines and caged coins are between the barriers. Additional viruses and caged coins are located below the columns with viruses and coins. The filled flasks in this stage will shoot out either viruses, shells or exploders.

Purple coins are unlocked in the following sequence:

  1. The 4 half-frozen coins in row #2.
  2. The 4 coins in row #10.
  3. The 4 coins in row #1, in between the barriers.
  4. The 4 coins in row #9.

In this stage, viruses are mainly found in the middle five columns, among four bubble machines and caged viruses and crates. On the leftmost column, there are lock blocks inside bubbles, while on the rightmost column, there are viruses inside bubbles. Dust viruses can be affected by bubble machines to remove the dust and become inside bubbles instead.

In this stage, the objects form a rotational symmetry, except for the lock blocks and the cages. Exploders are each located next to two purple coins and diagonally adjacent to a Brick Block and two viruses. All Brick Blocks contain coins.

Purple coins are unlocked in the following sequence:

  1. The coins in row #8 and the second coin in row #7, for a total of 3 coins.
  2. The coins in column #8 and the second coin in row #3, for a total of 3 coins.
  3. The coins in row #2 and the first coin in row #3, for a total of 3 coins.
  4. The coins in column #2 and the first coin in row #7, for a total of 3 coins.

In this stage, purple coins are placed in a cascading pattern, with a many of them half-frozen. Between the lines of purple coins, there are viruses and Capsule Plus arranged in "L" tromino patterns, with the pivot viruses being half-frozen.

  1. The coins starting from the last coin in row #9 to the last coin in row #7, for a total of 3 coins.
  2. The coins starting from the second coin in row #9 to the last coin in row #4, for a total of 6 coins.
  3. The coins starting from the first coin in row #9 to the last coin in row #1, for a total of 9 coins.
  4. The coins starting from the first coin in row #6 to the second coin in row #1, for a total of 6 coins.

In the final stage of this world and starting with 37 capsules, the objective is to eliminate all viruses (61) and collect all keys (3).

In this stage, the middle column contains viruses, keys and lock blocks inside bubbles. On the left and right sides in the beginning, crates, a few Empty Blocks and a few half-frozen bubble machines form a path containing viruses. After the stage scrolled up far enough, there exploders and doors in cages, along with several viruses above Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 14. Upon completion, players earn a free Staff Ticket.

In this stage, colorful crates arranged in a checkered pattern, and are found in barrels except for the bottom-most row of colorful crates. Viruses are found between the gaps of colorful crates.

Completing this stage awards the player 500 coins.

  1. The 3 coins in row #8.
  2. The 3 coins in row #7.
  3. The 3 coins in row #6.
  4. The 3 coins in row #5.
  5. The 3 coins in row #4.
  6. The 3 coins in row #3.
  7. The 3 coins in row #2.

Completing this stage awards the player 500 coins.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Starting with 45 capsules, the objective of this stage is to eliminate all viruses (13) and collect all keys (35). Unlike the rest of the special stages which have the available colors of pink, blue and yellow, the colors available in this stage are red, blue and yellow.

In this stage, keys are blocked by different things before the doors. In the lower rows, the doors are half-frozen. The following rows have hidden doors in Brick Blocks. Colorful crates are between the keys and doors in the next rows. The following rows have barriers between the keys and doors. Near the top of the stage, doors are locked behind cages with its corresponding lock block at the corner.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Fire Luigi.
