World 13 is an underground crystal cavern world resembling Tilted Tunnel from New Super Mario Bros. U in Dr. Mario World. The first area is available as of 1 AM PT on Mar. 5, 2020[1], while the second area, challenge stages and special stage are available as of 2 AM PT on Mar. 19, 2020[2].
When an area is cleared, Bony Beetles, Bone Goombas, and Swoops appear. The viruses featured in the regular stages are red, yellow and blue, with the exception of one stage, which has green viruses in addition to blue.

Starting with 38 capsules, the objective of this stage is to collect all coins from Coin Blocks (48), collect all virus coins (4) and collect all purple coins (12).
In this stage, the objects form a rotational symmetry, except for the lock blocks and the cages. Exploders are each located next to two purple coins and diagonally adjacent to a Brick Block and two viruses. All Brick Blocks contain coins. Purple coins are unlocked in the following sequence:

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (38) and break all colorful crates (15).
In this stage, all viruses are locked behind cages in a checkered pattern, except for the middle where there is a blue lock block and the top middle where a red lock block is in a cage. Colorful crates and a yellow lock block are situated between viruses. | |

Starting with 8 capsules, the objective of this stage is to eliminate all viruses (45) and collect all purple coins (27).
In this stage, purple coins are placed in a cascading pattern, with a many of them half-frozen. Between the lines of purple coins, there are viruses and Capsule Plus arranged in "L" tromino patterns, with the pivot viruses being half-frozen. Purple coins are unlocked in the following sequence:

Starting with 39 capsules, the objective of this stage is to eliminate all viruses (13), collect all purple coins (17) and collect all keys (2).
In the middle of the stage, a shell is in the same row as the red and yellow lock blocks, and a blue lock block is found in the top row in the middle. Some of the colorful crates and the purple coins are half-frozen. In addition, crates are found in the leftmost and the rightmost columns. Purple coins are unlocked in the following sequence:

In this stage, several bubble machines are located in the odd-numbered columns, most of them half-frozen. Exploders are found in row #6. All Brick Blocks contain coins. The filled flasks in this stage will shoot out exploders. | |

In the final stage of this world and starting with 37 capsules, the objective is to eliminate all viruses (61) and collect all keys (3).
In this stage, the middle column contains viruses, keys and lock blocks inside bubbles. On the left and right sides in the beginning, crates, a few Empty Blocks and a few half-frozen bubble machines form a path containing viruses. After the stage scrolled up far enough, there exploders and doors in cages, along with several viruses above Empty Blocks. This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced. Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 14. Upon completion, players earn a free Staff Ticket. |

Within a 70 second time limit, the objective of this stage is to eliminate all viruses (14), collect all purple coins (21) and collect all keys (3). The colors available in this stage are blue, green, and pink.
In this stage, the leftmost and rightmost keys and doors have columns of viruses between them, while the middle key and door have crates between them. In the other columns, purple coins in rows of three and colorful crates in rows of three are placed in alternate rows. Purple coins are unlocked in the following sequence:

Within a 90 second time limit, the objective of this stage is to eliminate all viruses (36) and collect all coins from Coin Blocks (21). The colors available in this stage are blue, green, and pink.
In this stage, the ring of barrels contain viruses, and the ring of dust contain barriers. Barriers in dust will not switch on or off. All Brick Blocks contain coins. Completing this stage awards the player 1000 coins and a Staff Ticket. |

Starting with 45 capsules, the objective of this stage is to eliminate all viruses (13) and collect all keys (35). Unlike the rest of the special stages which have the available colors of pink, blue and yellow, the colors available in this stage are red, blue and yellow.
In this stage, keys are blocked by different things before the doors. In the lower rows, the doors are half-frozen. The following rows have hidden doors in Brick Blocks. Colorful crates are between the keys and doors in the next rows. The following rows have barriers between the keys and doors. Near the top of the stage, doors are locked behind cages with its corresponding lock block at the corner. This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced. Completing this stage awards the player 1000 coins and Dr. Fire Luigi. |
