World 13 is an underground crystal cavern world resembling Tilted Tunnel from New Super Mario Bros. U in Dr. Mario World. The first area is available as of 1 AM PT on Mar. 5, 2020[1], while the second area, challenge stages and special stage are available as of 2 AM PT on Mar. 19, 2020[2].

When an area is cleared, Bony Beetles, Bone Goombas, and Swoops appear. The viruses featured in the regular stages are red, yellow and blue.

Area 1

Stage 481 from Dr. Mario World Stage 481 This stage introduces bubble machines, which will activate when a match adjacent to it is made, and will wrap a virus or certain objects in a bubble when another match adjacent to it is made. The objective of this stage is to eliminate all viruses. The Brick Blocks in the fifth row contain hidden viruses.
Stage 482 from Dr. Mario World Stage 482 The objective of this stage is to eliminate all viruses, collect all purple coins and collect all keys. In this stage, three rows containing four purple coins in bubbles are below three rows of viruses. Bubble machines can be found between the rows of viruses. Caged purple coins do not need to be unlocked to be able to pop the bubble.

Purple coins are unlocked in the following sequence:

  1. The purple coins originally found in the eighth row.
  2. The purple coins originally found in the sixth row.
  3. The purple coins originally found in the fourth row.
Stage 483 from Dr. Mario World Stage 483 The objective of this stage is to eliminate all viruses. In this stage, half-frozen viruses are found in a formation of a diamond with a bubble machine in the middle. Brick Blocks are also in the formation of a diamond, with half-frozen ones near ice fans. The ones that are not frozen contain hidden viruses. Additionally, there are two bubble viruses originally near the ice fans.
Stage 484 from Dr. Mario World Stage 484 The objective of this stage is to eliminate all viruses and destroy all Brick Blocks. In this stage, Brick Blocks behind cages are found on the three leftmost columns and three rightmost columns, with bubble viruses and the Lock Blocks corresponding to the opposite cages in between. The middle three columns just contain viruses.
Stage 485 from Dr. Mario World Stage 485 The objective of this stage is to collect all Virus Coins. On the three leftmost columns and the three rightmost columns, colorful crates are lined in four columns, along with Floatie Viruses and Brick Blocks containing hidden viruses between the columns of colorful crates. The middle columns contain viruses in groups of nine above a row of bubble machines. All Virus Coins are sized 3x3 and are found along the first to third rows and fifth to seventh rows.
Stage 486 from Dr. Mario World Stage 486 The objective of this stage is to eliminate all viruses and collect all purple coins. In this stage, caged purple coins surround barrels, with some partially revealing its contents that include exploders and viruses. All but one unbroken barrel contain viruses, in which a purple coins if found there instead. Filled flasks in this stage will shoot out either shells or viruses.

Purple coins are unlocked in the following sequence:

  1. The coin in the middle, undamaged barrel.
  2. The coins surrounding the barrels.
Stage 487 from Dr. Mario World Stage 487 The objective of this stage is to eliminate all viruses. In this stage, several viruses are locked behind cages, along with bubble machines. Every Brick Block contains a hidden Lock Block, with exploders next to the yellow one in the middle and the red and blue ones in the top row (left and right, respectively).
Stage 488 from Dr. Mario World Stage 488 The objective of this stage is to collect all purple coins and collect all keys. In this stage, viruses forming a diamond are shrouded in clouds, and are between keys and doors. The Brick Blocks from the seventh to tenth rows contain keys and the Brick Blocks from the second to fifth rows contain doors. A formation of purple coins are found next to the Brick Blocks containing doors.

Purple coins are unlocked in the following sequence:

  1. The coins in the fifth row.
  2. The coins in the fourth row.
  3. The coins in the third row.
  4. The coins in the second row.
  5. The coin in the first row.
Stage 489 from Dr. Mario World Stage 489 The objective of this stage is to eliminate all yellow viruses (eliminating red and blue viruses do not contribute to the objective). In this stage, the middle yellow viruses are locked behind cages and surrounded by yellow colorful crates and Empty Blocks in the middle three columns. In the other columns, yellow viruses are found in pairs next to Empty Blocks, except for one which is paired with a yellow Lock Block. Fully-frozen red and blue viruses are also available.
Stage 490 from Dr. Mario World Stage 490 The objective of this stage is to eliminate all viruses, destroy all colorful crates and collect all keys. In this stage, the middle column contains Brick Blocks and a door in bubbles, with the door starting from a position next to a column of crates until the end where it could be found between Brick Blocks. Colorful crates in groups of three-in-a-row are available, with viruses occupying the rest of the spaces except for the middle column.

This stage will scroll for one line each time enough objects are cleared, and will stop after 5 lines are introduced.

Stage 491 from Dr. Mario World Stage 491 The objective of this stage is to eliminate all viruses and destroy all barriers. This stage is shaped like a right-angle triangle, with crates, barriers shrouded in cloudes and dust containing Floatie Viruses. Filled flasks in this stage will shoot out either shells or crates.
Stage 492 from Dr. Mario World Stage 492 The objective of this stage is to eliminate all viruses and collect all keys. In this stage, doors and half-frozen keys are placed next to some colorful crates, with six of the doors being fully-frozen.
Stage 493 from Dr. Mario World Stage 493 The objective of this stage is to eliminate all viruses and destroy all Brick Blocks. In this stage, fully-frozen Brick Blocks form a boundary that surround the viruses in it. There are viruses outside the boundary that are also next to bubble machines. Ice fans are found in the top row of the stage.
Stage 494 from Dr. Mario World Stage 494 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and destroy all crates. In this stage, Brick Blocks each containing coins are found between viruses at the top and bottom in groups of columns of three, which are all between crates in groups of three in a column in a cascading fashion.
Stage 495 from Dr. Mario World Stage 495 The objective of this stage is to eliminate all viruses and destroy all barriers. In this stage, barriers are found next to groups of caged viruses, with a Lock Block found at the bottom surrounded by barriers and cages and at the top locked behind a cage.
Stage 496 from Dr. Mario World Stage 496 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and collect all purple coins. In this stage, half-frozen viruses are found in the middle surrounded by Brick Blocks. The leftmost column, rightmost column, top row and bottom row of Brick Blocks contain coins.

Purple coins are unlocked in the following sequence:

  1. The coins in the leftmost and rightmost columns.
  2. The coins inthe topmost row and the four coins at the seventh and eighth rows.
  3. The rest of the coins found in the second and third rows.
Stage 497 from Dr. Mario World Stage 497 The objective of this stage is to eliminate all viruses and destroy all crates. In this stage, crates in groups of three in a column are found in the first three rows, with viruses and exploders between them. Three bubble machines are found in the eighth row. Each column also has one bubble, either encasing Brick Blocks or viruses.
Stage 498 from Dr. Mario World Stage 498 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and collect all purple coins. In this stage, two shells are found in the middle column, where they are aligned with viruses and purple coins. Clearing the blue Lock Block unlocks cages containing viruses in the rows containing Empty Blocks while clearing the red Lock Block unlocks the cages containing Brick Blocks containing pink coins.

Purple coins are unlocked in the following sequence:

  1. The coins in the same rows as the shells.
  2. The coins in the leftmost and rightmost columns.
  3. The coins in the third and seventh rows.
  4. The rest of the coins in the second and eighth rows.
Stage 499 from Dr. Mario World Stage 499 The objective of this stage is to eliminate all yellow viruses (eliminating red and blue viruses do not contribute to the objective) and destroy all barriers. In this stage, the yellow viruses are all found in undamaged barrels, each of them directly below a barrier. Between barrels, red and blue Floatie Viruses are found.
Stage 500 from Dr. Mario World Stage 500 The last stage of the first area, the objective of this stage is to eliminate all viruses and collect all purple coins. In this stage, all colorful crates are wrapped in bubbles, with several of them floated upwards toward viruses. The initial three rows and the three rows after that also contain half-frozen coins and viruses covered in dust. Brick Blocks do not containing anything.

This stage will scroll for one line each time enough objects are cleared, and will stop after 7 lines are introduced.

Purple coins are unlocked in the following sequence:

  1. The coin initially at the third row.
  2. The coin initially at the second row.
  3. The coin initially at the first row.
  4. The coins after the stage scrolls 3 lines, each revealed row containing one coin.
  5. The coins that appear after the stage scrolls 6 lines.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Bony Beetle assistant upon completing this stage.

Area 2

Stage 501 from Dr. Mario World Stage 501 The objective of this stage is to eliminate all viruses, including any that appears from flasks. In this stage, there are two medium-sized flasks and two rows of Empty Blocks separating two groups of barrels each arranged in a zig-zag line, with viruses found within the right-angled curves. Four more viruses are found in barrels in the top half, while the rest of the barrels contain exploders. Filled flasks in this stage will shoot out viruses.
Stage 502 from Dr. Mario World Stage 502 The objective of this stage is to collect all keys. In this stage, bubble viruses and colorful crates are arranged in diagonal lines, with every colorful crate found in partially damaged barrels except for the ones in the bottom-most row.
Stage 503 from Dr. Mario World Stage 503 The objective of this stage is to eliminate all viruses and destroy all barries. In this stage, barriers found in barrels form two overlapping circles. Barriers found in barrels will not change state until the barrel is broken.
Stage 504 from Dr. Mario World Stage 504 The objective of this stage is to eliminate all viruses. In this stage, two groups of six viruses are surrounded by crates and a few colorful crates, and one group of virus is behind colorful crates and Empty Blocks. Additionally, seven Capsule Plus capsules are found throughout the stage, one of which is next to one group of viruses surrounded by crates.
Stage 505 from Dr. Mario World Stage 505 The objective of this stage is to eliminate all viruses and collect all pink coins from Brick Blocks. a row of eight red Floatie Viruses are found between a row of Brick Blocks and red colorful crates, and four additional Floatie Viruses are found above the group of Brick Blocks. Colorful crates are lined up on the sides above the group of Brick Blocks, along with two in the middle that are surrrounded by viruses. Every Brick Block contains coins.
Stage 506 from Dr. Mario World Stage 506 The objective of this stage is to eliminate all viruses and collect all keys. In this stage, viruses, half-frozen barriers and Brick Blocks intersect with one another. The Brick Blocks in the topmost row contain doors, while the rightmost Brick Block in the bottom-most row contains a key. The Brick Blocks surrounded by viruses contain hidden viruses except for two, which contain keys.
Stage 507 from Dr. Mario World Stage 507 The objective of this stage is to collect all Virus Coins. This stage is split into nine sections of three rows and three columns separated by colorful crates, and each section contains a Virus Coin. Several of the colorful crates are locked behind cages with its corresponding Lock Block in the middle section. Filled flasks in this stage will shoot out colorful crates.
Stage 508 from Dr. Mario World Stage 508 The objective of this stage is to eliminiate all viruses and collect all purple coins. In this stage, two large groups of purple coins are found, separated by a line of half-frozen viruses. Next to each group are three shells and three Floatie Viruses. The Brick Block has a hidden purple coin. The order in which the coins are unlocked are the following:
  1. Two groups of six coins at the bottom half of the group and one coin in the Brick Block
  2. Two groups of six coins at the top half of the group
  3. Six coins that divided the two groups of six coins in both halves
  4. The remaining coins that surrounded the other coins (except for the one in the Brick Block)
Stage 509 from Dr. Mario World Stage 509 The objective of this stage is to eliminate all viruses and collect all pink coins from Brick Blocks. In this stage, a group of viruses covered in dust are found in the middle, and a Lock Block locking colorful crates shaped in an upside-down "U" is found above a group of viruses. All Brick Blocks except for those in the same row as the Lock Block contain coins.
Stage 510 from Dr. Mario World Stage 510 The objective of this stage is to eliminate all viruses. In this stage, a large group of viruses in a shape of a filled "8" is found next to crates. Filled flasks in this stage will shoot out crates of variable durabilities.
Stage 511 from Dr. Mario World Stage 511 The objective of this stage is to collect all purple coins and collect all keys. In this stage, purple coins surround two groups of 3x5 viruses, one group being half-frozen and another group wrapped in bubbles. Purple coins can be found in broken and unbroken barrels, with some of them in the way between the keys and doors.
Stage 512 from Dr. Mario World Stage 512 The objective of this stage is to eliminate all viruses. In this stage, colorful crates separated groups of viruses, along with a few individual viruses near Empty Blocks. The red Lock Block is surrounded by Empty Blocks, along with Exploders near the Lock Block. The dust hides viruses.
Stage 513 from Dr. Mario World Stage 513 The objective of this stage is to eliminate all viruses and collect all purple coins. In this stage, bubble viruses are found above purple coins found below crates, while caged purple coins are found above crates. A line of viruses are found directly below ice fans at the top of the stage. Purple coins are unlocked in the following sequence:
  1. The nine coins found along the bottom of the crates
  2. Coins found in the fourth and sixth columns
  3. The rest of the coins along the fourth row
Stage 514 from Dr. Mario World Stage 514 The objective of this stage is to eliminiate all viruses and destroy all barriers. In this stage, viruses are lined up vertically, save for a few lined up horizontally along the barriers. All Brick Blocks contain hidden viruses. A Capsule Plus is found in a partially damaged barrel at the top-middle part of the stage.
Stage 515 from Dr. Mario World Stage 515 The objective of this stage is to eliminate all viruses. In this stage, colorful crates form two horizontal lines and two vertical lines criss-crossed with each other, with viruses occupying the spaces in between, along with colorful crates shaped like a cross at the top-right corner separated by ice fans and a lone virus.
Stage 516 from Dr. Mario World Stage 516 The objective of this stage is to eliminate all viruses. In this stage, four lines containing Empty Blocks and crates are found separated by a column of bubbles in the middle, holding viruses, Floatie Viruses and a flask. Additional viruses are found along the lines of crates and Empty Blocks. The filled flask in this stage will shoot out bubble viruses.
Stage 517 from Dr. Mario World Stage 517 The objective of this stage is to eliminate all viruses, collect all pink coins from Brick Blocks and collect all purple coins. In this stage, the layout is symmetrical, save for the colors of the viruses and shells. Lock Blocks are found in the middle column, with crates behind the blue cages, Brick Blocks behind the yellow cages and viruses behind the red cages. All Brick Blocks contain coins. The order in which the purple coins are unlocked are as follows:
  1. Single coin on the leftmost column
  2. Single coin on the rightmost column
  3. Coins from both the second and eighth columns
  4. Rest of the coins found in the sixth row
Stage 518 from Dr. Mario World Stage 518 The objective of this stage is to collect all keys. In this stage, the viruses form a shape of a bunny's head. Among the viruses, four Floatie Viruses of each of the three colors are found. Doors are found in the topmost row, while keys are initially located below the yellow viruses.
Stage 519 from Dr. Mario World Stage 519 The objective of this stage is to eliminate all viruses and collect all purple coins. In this stage, half-frozen viruses, exploders, shells and Brick Blocks are found, along with a few fully-frozen Brick Blocks and purple coins. The order in which the purple coins are unlocked are as follows:
  1. The two unfrozen coins and three fully-frozen coins
  2. The two middle coins in the fourth row
  3. The coins found in the middle three columns
  4. The rest of the coins found in the leftmost two columns and rightmost two columns
Stage 520 from Dr. Mario World Stage 520 The final stage is a scrolling stage that requires all viruses to be eliminated. In this stage, various Floatie Viruses are found in the beginning, along with Brick Blocks and viruses in barrels throughout the stages. Bubble viruses appear eventually, and later ice fans start to appear. Exploders later appear, and a couple of half-frozen viruses one line after that. A line of regular viruses are found near the top, with the top containing a line of Empty Blocks. This stage will scroll for one line each time enough objects are cleared, and will stop after 10 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 14. Upon completion, players earn a free Staff Ticket that lets them unlock either a new doctor, a new assistant, or leveling up an existing doctor or assistant.

Challenge stages

Stage 13A from Dr. Mario World Stage 13A Within a 70 second time limit, players need to eliminate all viruses and destroy all colorful crates. In this stage, colorful crates arranged in a checkered pattern, and are found in barrels except for the bottom-most row of colorful crates. Viruses are found between the gaps of colorful crates.

Completing this stage awards the player 500 coins.

Stage 13B from Dr. Mario World Stage 13B Within a 70 second time limit, players need to eliminate all viruses, collect all purple coins and collect all keys. In this stage, the leftmost and rightmost keys and doors have columns of viruses between them, while the middle key and door have crates between them. In the other columns, purple coins in rows of three and colorful crates in rows of three are placed in alternate rows.

Purple coins are unlocked in the following sequence:

  1. The coins in the eighth row.
  2. The coins in the seventh row.
  3. The coins in the sixth row.
  4. The coins in the fifth row.
  5. The coins in the fourth row.
  6. The coins in the third row.
  7. The coins in the second row.

Completing this stage awards the player 500 coins.

Stage 13C from Dr. Mario World Stage 13C Within a 90 second time limit, players need to eliminate all viruses and collect all pink coins from Brick Blocks. In this stage, the ring of barrels contain viruses, and the ring of dust contain barriers. Barriers in dust will not switch on or off. All Brick Blocks contain coins.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Special Stage

World 13's Special Stage from Dr. Mario World Special Stage The objective of this stage is to eliminate all viruses and collect all keys. In this stage, keys are blocked by different things before the doors. In the lower rows, the doors are half-frozen. The following rows have hidden doors in Brick Blocks. Colorful crates are between the keys and doors in the next rows. The following rows have barriers between the keys and doors. Near the top of the stage, doors are locked behind cages with its corresponding Lock Block at the corner. Unlike the other special stages, which has pink, blue and yellow viruses, this special stage contains red, blue and yellow viruses.

This stage will scroll for one line each time enough objects are cleared, and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Fire Luigi.
