The Miracle Kinoko is a giant, glowing, spotted mushroom that is located in the Kinoko Yama region, where it is considered the protective deity from the locals. It appears in the Dr. Mario manga, where it is explained it is the main component of a capsule.

After noticing it that the mountain region of the Mushroom Kingdom is uninfected from the Gahaha Biyō, Mario, Princess Peach, and Princess Daisy go to investigate, and are ambushed by Gahaha Daiō. Escaping with the locals, the group hides behind the Miracle Kinoko, and they discover that is the reason behind the virus immunity, seeing as it repels both the virus and the Dokudoku Pakkun Flower. After the village chief gives a bite of it to the princesses, who were infected, Mario creates a cure out of it and uses it to defeat Gahaha Daiō and cure the Kingdom.

With one last Gahaha Virus remaining, Gahaha Daiō transforms the germ in the Red, Blue, and Yellow Viruses. In doing so, Bowser has decomposed their weakness to the Miracle Kinoko, making them immune to it, so when Mario goes to cure Luigi with the mushroom, it is rejected. At first, Mario and Peach try to decompose the Miracle Kinoko bio-structure to adapt to the Virus' evolution, but later they discover that their weakness is aligning their colors in groups of four.
