Spaghetti is a type of pasta in the real world, and an item found in the Paper Mario series, Spaghetti is also Mario's favorite food as said in The Super Mario Bros. Super Show! and officially by Nintendo. It was briefly mentioned in the opening of Hotel Mario as well.
In Super Mario 64, if Mario is left idle long enough he will take a nap and can be heard talking in his sleep, during which time he mention the italian foods spaghetti and ravioli (leaving one to assume he is dreaming about them).
In the Paper Mario series, Tayce T. can make it with Dried Pasta, but Zess T. requires Fresh Pasta. It heals six HP and four FP, and also increases defense that turn by one if used in battle.
A similar item called Spaghetti Plate appears in Super Paper Mario.
The item doubles in healing power when cooked with Dried Pasta, but loses healing power when cooked with Fresh Pasta.
