So this is a bit of a weird one. This is, by all accounts, not a glitch. It fails pretty much every point made in the "Legitimacy of a glitch" section of MarioWiki:Glitches; it is a rather standard disk error screen with no known way to trigger it on an actual copy without the game being tampered with, whether the disk is ejected during gameplay or damaged/corrupted in other ways.
However, as Camwoodstock pointed out when they reverted my initial attempt to delete it, this is not an entirely obscure error, due to the fact that most roms of the PC-88 version floating around are of a bad dump that trigger it when trying to load World 8-4. Coupled with the rarity of an actual copy and the lack of an official rerelease or even mention of any kind, and one of the most popular let's plays of the game (which is linked to in the error's entry on the page) initially playing on this rom and triggering this error, it is one that quite a bit of the western side of the web who know of this game are aware of.
Therefore, I do still think this error deserves coverage due to its rather unique history, with no other Mario game having had a bad dump with a game-breaking error be this widely circulated to my knowledge, just not in the glitches section, since it again fails our glitch policies (And on another note, general disk error screens aren't really something you could consider glitches by any stretch either. We wouldn't list the Wii's disk error message in the list of Super Mario Galaxy glitches either). I thereby propose moving it to the Notes section of the main page, which I feel is the most fitting place to put something like this, though I have also included an option to remove the mention entirely.

Move to Notes section of main page[edit]
- Snessy (talk) Per proposal.
- Camwoodstock (talk) A move to the notes section feels fair to us. It is a notorious issue that has plagued the majority of folks that are playing this game in the modern day, though calling it a glitch is, admittedly, a bit of a misnomer.
- EvieMaybe (talk) per proposal
- The Dab Master (talk) Per proposal.
- ALTENDO VOTE ERROR! • PLEASE REDO EDIT • PRESS SAVE CHANGES • This is an error, not a glitch. Players cannot even try to get this glitch; all that happens is that bad luck can create this error, not a player exploiting the game.
- Maw-Ray Master (talk) Per all.
