Players can jump on their opponents to stun them temporarily. Players who are hit by Bullet Bills, fall off the platforms and touch the ground, or stay on platforms that lower are eliminated. Players who reach the end of the obstacle course win. If all players are eliminated, the minigame ends in a tie.

Sick and Twisted is a 4-Player minigame that is found in Mario Party 8. The name of this minigame comes from a phrase that is said about a person who is exceedingly mean and mad.
