The Sapphire Star is the fifth Crystal Star found in the Pirate's Grotto and is one of Cortez's treasures  in Paper Mario: The Thousand-Year Door. Cortez gives this Crystal Star to Mario when Mario tells the undead pirate he is not interested in his treasure, only the Star. Cortez hands it over willingly, stating that he didn't even particularly like the Star.

The Sapphire Star is the fifth of the Crystal Stars Mario collects in Paper Mario: The Thousand-Year Door, obtained after finishing Chapter 5: The Key to Pirates.
The Sapphire Star teaches Mario the move Sweet Feast, which costs five SP. As opposed to Sweet Treat, the player will simply have to hold left instead of repeatedly tapping it. Mario can gain much more HP and FP using this, but Poison Mushrooms are still a threat and appear more frequently.

According to the legend of the Crystal Stars as told by Grifty, the Sapphire Star was brought to Keelhaul Key by the tired Koopa hero in an effort to keep it safe. However, the pirate Cortez stole the star from him easily and added it to his treasure hoard. After being shipwrecked on Keelhaul Key and navigating Pirate's Grotto, Mario encounters Cortez and battles him for the Sapphire Star. After the battle, Cortez gives it up willingly, stating he didn't even like it nor need it. The Sapphire Star teaches Mario the move Sweet Feast to use in battles.

During the final battle with the Shadow Queen, when the Crystal Stars spread out to the locations in which they were found, the Sapphire Star returns to Keelhaul Key, where Pa-Patch and the rest of the shipwrecked crew lend their support to Mario and his team just before the final round.
