You're right about the title card. I'm assuming the name comes from the background of the tech demo, which reads "Super Mario 128". If it was supposed to lead into the next mainline platformer, "Super Mario 128" would follow the naming convention of the series. Everyone referring to it as "Mario 128" is likely just a time-saving measure, similar to how SM64 was shortened to "Mario 64" in Odyssey.
