In Luigi's Mansion, Boolossus, along with the other Boos (including King Boo), has decided to take a rest in a secret area under the Storage Room. When Luigi activates a hidden door in the Storage Room while searching for his brother, the Boos spring out, enraged after being awakened. However, when they notice E. Gadd's Poltergust 3000, a ghost-catching vacuum device, on Luigi's back, the Boos, including Boolossus and King Boo, decide to run off and hide in the mansion instead of challenging the novice ghost hunter.

Eventually, as Luigi progresses through the mansion, Boolossus attempts to capture the green-clad plumber on behalf of his leader, King Boo. When Luigi reachs the Balcony on the third floor, the Boo decides to make his move. While on the Balcony, Luigi notices that the Boo Radar on his Game Boy Horror has started flashing, indicating a powerful Boo presence. Soon, the fifteen Boos that make up Boolossus appear before Luigi, circling and taunting the novice ghost hunter. After having their fun, they to get serious by forming into the gigantic Boolossus and whisking Luigi away to the balcony arena, starting the battle to capture the green-clad hero and turn him into a portrait, just like his brother Mario.

When Luigi reachs the Balcony on the third floor, the Boo decides to make his move. While on the Balcony, the Boo Radar on the Game Boy Horror starts flashing intensely, indicating a powerful Boo presence.

During the battle, Boolossus attempts to slam into Luigi and cause massive damage. To defeat the giant Boo, Luigi must use his Poltergust 3000 to pull Boolossus into one of the two conveniently placed unicorn ice sculptures located on the balcony. When Boolossus touches the sharp horn of the statue, he pops and separates into the fifteen normal Boos. Luigi may then absorb an Ice Elemental Ghost from the ice sculpture and spray frost at the Boos, freezing the ghosts and enabling him to suck them up, one by one, into his Poltergust 3000. Eventually, Boolossus reforms with a lower number of Boos. Although smaller in size than before, he resumes the fight against the green-clad hero. He becomes faster every time some of the Boos are sucked up. However, if Luigi keeps popping the big Boo and freezing the small Boos, he can slowly drain Boolossus's power. As more and more Boos are caught, the other ghosts become more wary of the Poltergust's ice blasts. As such, they have to be carefully pulled closer to Luigi's position with the vacuum device before shooting them with a sudden stream of icy mist.
