The level begins with Mario Ground Pounding into the cave, breaking a few Brick Blocks directly below him and collecting some coins. Right after this are two ? Blocks, one of which contains either a Super Mushroom or a Fire Flower, depending on Mario's current form. There are also additional coins that can be collected with more Ground Pounding. Just after this, a Goomba appears, the first of many throughout the level. The first Star Coin can also be located nearby in an alcove.

The level begins with Mario ground pounding into the cave. Many Goombas and Koopa Troopas are found in this cave. Mario encounters some ? Blocks. Hitting them produces Coins. Mario collects them and moves on to the first Star Coin. He can jump onto the ledge, and collect it. He can also Ground Pound on some bricks to collect a bunch of coins. He then moves on, encountering more Goombas. Going forward, Mario finds a block of rock that he can get through by smashing the bricks above it. An abyss is found, which Mario can jump over it, followed by the second Star Coin, surrounded by some bricks. Mario can collect the Star Coin by ground pounding the blocks above it. A Piranha Plant is then met. There is a Super Star above the first brick after the first Piranha Plant. Mario jumps over the Piranha Plant and goes down the pipe it was guarding. Mario encounters some Coins, and a Koopa Troopa can be used to break some bricks, creating a passageway. Mario crawls under the passageway, where some Goombas are found. Running, he then sees a ? Block, which contains a Mega Mushroom. Mario can use it to destroy all the Goombas and bricks. He then goes up a pipe nearby, leading him back to the main cave. He must jump over some pipes sticking out of an abyss. He then makes it onto the ground and runs forward. He then encounters a seesaw. He runs over it, and continues through the cave. A pipe is found at the end, which leads outside, near the pole. He then finishes the level.
After this are more enemies, including Koopa Troopas, as well as coins. There is a long row of Brick Blocks overhead, one of which contains a 1-Up Mushroom. The second Star Coin appears just after this, surrounded with blocks that can be broken either by Ground Pounding or with a Koopa Troopa's shell. After a seesaw is the level's Checkpoint Flag.

Up next are Piranha Plants in pipes. A Hidden Block containing a Super Star can be found in this area. The first pipe can also be entered to find a secret area with a few extra coins, as well as a red Koopa Troopa whose shell can be used to clear away a row of Brick Blocks below, revealing another hidden area filled with Goombas, as well as a ? Block containing a Mega Mushroom.

Back in the main area, Mario must traverse a few more obstacles in the level, including enemies and a seesaw, before entering a Warp Pipe to return to the surface, where the Goal Pole allows him to finish the level. Alternatively, in a nod to World 1-2 from the original Super Mario Bros., Mario can continue upward and to the right to find the level's third and final Star Coin and a pipe leading to a secret Goal Pole.

Red-flag exit
Red-flag exit
Towards the end of the level, there will be a small gap at the top of the screen like World 1-2 in the original Super Mario Bros. that Mario can access using the seesaw. Running across the gap will lead to the third Star Coin and a pipe, which will lead to the red-flag exit. Using the red-flag exit unlocks a different path that directly leads to a secret Toad House and the tower of that world.

Star Coins
*Star Coin 1 - In the beginning of the level, it can be found on a ledge that needs the use of bricks to get it.
*Star Coin 2 - A huge plethora of blocks surrounds this coin. Ground pounding the top of the box will reveal an opening.
*Star Coin 3 - On the way to the secret exit, the Star Coin should be in plain sight.
