(Well they are definitely not a Mega Mole due to Mega Moles having long noses and this Monty Mole having the Monty Mole nose. I could make a poll discussing wether or not these two are the same.)

While traversing the Jungle for the kart race, Mario, Luigi and Peach are ambushed by Choropū-san, who tosses a Kurayami sasori that stings Luigi. After hearing Dr. Mario's diagnosis, Mario learns that the Kurayami sasori venom kill the victim in 30 minutes (and they already wasted 5), but Dr. Mario also adds that the Amazon Queen should know where to find the antidote.

13 minutes later, Mario, Peach and Luigi (with a very swollen nose) reach the Queen at the jungle volcano, which explains the antidote is in the Pū Tairiku, a dangerous place from which no one came out alive, and whose entrance is situated right next to the Amazon Kingdom's. After opening it, the Queen informs them that they won't be able to exit through the same way they got in. Meanwhile, the rest of the racers see Mario's kart entering a cave, and they all think he went for a shortcut (Mario and Peach don't notice them with Luigi's swollen nose obstructing the back view). Entering in town, Mario's kart is attacked by a Monty Mole who looks like Choropū-san who reveals to be the warden, mentioning also that the antidote is in the inner temple. Mario runs him over and drive for the temple, followed by the other racers.

Entering in town, Mario's kart is then attacked by Choropū-san who reveals to be the last guard of the inner temple, mentioning also that the antidote is in there and having to defeat him in order to get to it. While crossing a frail bridge, Mario makes it crumble, making fall the other contestants down the below river. Only Dr. Mario and Nurse Peach kart manages to pass over, wanting to help Mario (and win the race money prize for a new hospital). With 3 minutes left for Luigi, the two teams reach the inner temple (a massive statue of a Mega Mole holding a big tube containing the antidote). While Mario and Dr. Mario races to the top, Bowser and Wario arrives too, complicating things for them. In the end, Mario reaches the tube, but pulled by Wario, the tube slip off his hands and fall right on Luigi, curing him and shrinking his swollen nose.
