World 3 is a beach world in Dr. Mario World. This world introduces Floatie Viruses, which must be matched three times and move to a random spot whenever matched. The second area introduces viruses fully encased in ice.

When an area is cleared, Huckit Crabs and Goombas appear in the overworld. The viruses featured in the regular stages are red, yellow and light-blue.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (18).

This stage introduces Floatie Viruses, which teleport to another part of the stage when matched. There are three of them at the bottom, with Viruses and Brick Blocks arranged in a checker pattern above. The brick blocks are all empty.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (9).

In this stage, floatie viruses in rows of three are lined up with Brick Blocks and shells, while two other shells are lined with two exploders each.

Since March 18 2021, this stage is revamped, and the capsule amount is reduced from 38 to 30. Initially, this stage features small rows of viruses between Empty Blocks and brick blocks. There are two Lock Blocks that unlock rows of viruses, some with floaties.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (31).

This stage has a circle of Empty Blocks and Brick Blocks, with three floatie viruses below it. Two sides of the circle have viruses around them, with a skill charger in the center.

The capsule amount is increased from 32 to 36 since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 36 to 32.
  • The Empty Blocks in rows #8 and #9 are changed to Brick Blocks, along with the second Empty Block in row #7.

Starting with 37 capsules, the objective of this stage is to eliminate all viruses (22).

A horizontally-oriented stage with many brick blocks and sets of bubble viruses around them.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 35 to 37.
  • The available colors are changed to be red and yellow only.
  • All Empty Blocks are replaced with Brick Blocks.
  • Exploders replace the first and last viruses in row #1 and the middle virus in row #2.
  • The viruses in row #6 are changed to floatie viruses (still inside bubbles).
  • The 2 floatie viruses in row #7 are changed to viruses (still inside bubbles) while floatie viruses inside bubbles are added two spaces directly below each of them.

Starting with 33 capsules, the objective of this stage is to collect all coins from Coin Blocks (45).

A stage made up of diagonal lines of viruses and brick blocks that the player must get coins from.

The capsule amount is increased from 26 to 30 since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 30 to 33.
  • The first viruses in rows #1 and #3 are removed.
  • 2 exploders are added next to the Brick Block in the top-left corner.

This stage has sets of viruses between sets of blocks on the sides, with a column of floatie viruses in bubbles blocked off by a row of brick blocks in the middle. A pop cannon is also found in the middle column.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 39 to 35.
  • A pop cannon replaces the middle Brick Block in row #2.
  • All Empty Blocks are replaced with Brick Blocks.

Starting with 32 capsules, the objective of this stage is to collect all virus coins (4).

A stage made up of three rows of viruses and blocks. The top and bottom rows have ice viruses surrounding an Exploder, while the middle row is made up of many regular viruses with two empty blocks. Each of the groups that contain an exploder surrounded by viruses have a virus coin behind them.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The objective of this stage is changed from eliminating all viruses (38) to collecting all virus coins (4).
  • The Empty Blocks in rows #2 and #9 are replaced with Brick Blocks, while the other Empty Blocks are replaced with exploders.
  • Virus coins are added behind each exploder and its surrounding viruses.

Starting with 36 capsules, the objective of this stage is to eliminate all viruses (31).

This stage has several brick blocks, with shells that can remove blocks, allowing bubble viruses and bubble lock blocks on the sides to float up. Exploders are found near the upper corners. Caged viruses are found at the bottom of the stage.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • This stage is no longer labeled as "Hard".
  • Exploders are added in the second and second-last spaces in row #2.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (22) and collect all coins from Coin Blokcs (42). The only color available in this stage is light-blue.

In this stage, there is a huge set of brick blocks with exploders. There are shells on the right that can activate the exploders, breaking the brick blocks. This reveals spread-out blocks with coins, with the other blocks breaking.

The capsule amount is increased from 23 to 26 since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 26 to 25.
  • The objective of this stage is changed to require eliminating all viruses in addition to collecting all coins from Coin Blocks.
  • The available color is changed to be light-blue only.
  • The virus in the top-right corner is removed, while viruses are added in row #9 to be in the same columns as the viruses in row #1.
  • Floatie viruses are added inside the Brick Blocks next to the exploders at rows #3 and #7.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (24).

This stage has columns of floatie viruses inside bubbles enclosed in empty blocks, blocked off by brick blocks. There are two sets of viruses surrounding an empty block at the top. On the leftmost and rightmost columns there is an exploder in each column.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • All Empty Blocks are replaced with Brick Blocks.
  • In rows #4 and #6 of the leftmost and rightmost columns, each side is filled with an exploder and a bubble containing a floating virus respectively.

Starting with 37 capsules, the objective of this stage is to eliminate all viruses (55). The only colors available in this stage are red and light-blue.

A stage with a large group of ice viruses in a diamond shape, with caged viruses surrounding them. A shell is in the center, with an exploder on each side. There are lock blocks at the bottom, with two of them caged.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 45 to 37.
  • The available colors are changed to red and light-blue only.
  • The red lock block is moved from the cage to the middle of row #7, replacing the virus (the ice is still there). In its original place is a virus. The yellow lock block is replaced with a virus.

Starting with 37 capsules, the objective of this stage is to eliminate all viruses (39).

A stage with many bubble viruses and brick blocks arranged in upside-down L shapes.

Prior to version 1.3.5, the stage layout was different. The following changes were made:

  • The visible virus in the third row is changed to a shell.
  • The third, fifth and seventh Brick Blocks in the third row no longer contain hidden viruses.
  • The second and fourth Brick Blocks in the fifth row no longer contain hidden viruses.
  • The second Brick Block in the seventh row no longer contains a hidden virus.
  • The viruses that were inside bubbles are no longer inside bubbles but are moved to the first row.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 44 to 37.
  • All Empty Blocks are replaced with Brick Blocks.
  • A shell replaced the leftmost virus in the topmost row.
  • An exploder is hidden inside the third Brick Block in row #5, replacing the virus hidden there.
  • The second Brick Block in row #3 no longer contains a hidden virus, but the second Brick Block in row #7 now contains a hidden virus.

A stage with a large set of brick blocks containing viruses, caged viruses, and ice viruses each.

  • Capsule amount is reduced from 40 to 34.
  • The virus colors below the lock block are changed to match its yellow color.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (49) and destroy all Brick Blocks (19). The only colors available in this stage are red and yellow.

In this stage, more of the stage comes in when enough of it is cleared. At the bottom is a row of bubble viruses, with viruses and brick blocks above, with a few floatie viruses at the top. Soon, a lock block will come in, with caged viruses on the sides.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 4 lines are introduced.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • This stage is now labeled as "Hard".
  • Capsule amount is reduced from 52 to 40.
  • The objective of this stage is changed to require destroying all Brick Blocks in addition to eliminating all viruses.
  • The available colors are changed to red and yellow only.
  • Brick Blocks are added to the leftmost and rightmost spaces in rows #3 and #5, as well as the second and second-last spaces in row #4.
  • A shell replaces a virus in the topmost row (before the stage scrolls).
  • In the topmost row after the stage scrolls all the way, a shell replaces the virus in the third cage and an exploder replaces a virus in the rightmost cage. All the caged viruses are changed to floatie viruses.
  • The row of Empty Blocks are removed from the topmost row, rendering the stage with only 4 rows to scroll through instead of 5.

  • Capsule amount is reduced from 45 to 25.
  • The available colors are changed to red and light-blue only.
  • Exploders replaced the first two viruses in row #6, the first three viruses in row #9, the second virus in row #5, the third virus in row #4 and all 3 viruses in the rightmost column. Of the exploders, all but the first two groups are fully-frozen.
  • All viruses that are not already fully-frozen are changed to be fully-frozen.

A stage with four sets of viruses, with the ones in the top-left and top-right being fully-frozen while the bottom-left and bottom-right being half-frozen. Each group has two exploders within them. Two skill chargers are located in the topmost row.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 42 to 32.
  • The available colors are changed to red and yellow only.
  • The Empty Block in row #9 is removed.
  • The bubble that contained a shell is gone, leaving only the shell.
  • Exploders are added in the gaps in row #3, and replaced the middle viruses within their groups in row #7.
  • The viruses that were hidden in Brick Blocks are changed to become fully-frozen.
  • The viruses that were not already frozen are changed to become half-frozen.

  • The Brick Blocks in between the light-blue lock block have their contents changed, where they originally had viruses. The one on the left now contains an exploder while the one on the right not contains a shell.

Starting with 39 capsules, the objective of this stage is to eliminate all viruses (35) and collect all Virus Coins (8).

A stage with two sets of ice viruses in the middle, along with a column of floatie viruses and brick blocks on the left.

  • This stage is no longer labeled as "Hard".
  • Capsule amount is reduced from 48 to 39.
  • The objective of this stage is changed to require eliminating all viruses in addition to collecting all virus coins.
  • The floatie viruses are changed to become inside bubbles, including a newly-added floatie virus in column #2.
  • In each group of viruses within the middle columns, 2 exploders are added in the inner ring, replacing the fully-frozen viruses. Morever, each outer ring now has only one color of virus instead of two.
  • The virus coins behind each group of viruses within the middle columns have a size increase from 4x4 to 5x5.

  • The available colors are changed to red and yellow only.
  • The Brick Blocks in the leftmost and rightmost columns are replaced with viruses.
  • The second-last virus in row #6 and the third-last virus in row #4 are each replaced with a shell.

  • The overall stage layout is vertically mirrored.
  • An exploder replaced a virus in the corner of red cages.
  • The light-blue lock block replaced the bubble containing a virus, and in its original place is a shell.
  • The Empty Blocks in the topmost row are changed to Brick Blocks except for the leftmost one (position after the mirroring).

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (35).

A vertically-oriented stage with many ice viruses and caged viruses.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 41 to 32.
  • All Empty Blocks are replaced with Brick Blocks.
  • The columns of virus above the gaps in columns #3, #5, #7 and #9 are changed to become caged instead of being half-frozen.
  • The red lock block is shifted from the top of the column of cages to the bottom of it.
  • The fifth cage in the leftmost column and the second-last cage in the rightmost column have pop cannons instead of viruses.

This stage has two sets of blocks with viruses surrounding them, and two lock blocks in the corners.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 39 to 25.
  • The half-frozen viruses directly diagonal from the lock blocks are changed so that they are not frozen, while the half-frozen viruses two spaces diagonally away from the lock blocks are changed to skill chargers.
  • The Empty Blocks near the cages are replaced with Brick Blocks.
  • The first cage in row #3 and the second cate in row #8 are changed to contain exploders instead of viruses.

The right side of the stage is made of caged viruses, and the left side is all brick blocks with three lock blocks and two floatie viruses.

The capsule amount is increased from 43 to 46 since version 1.3.5. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 43 to 37.
  • The third Brick Block in row #6 is replaced with a shell.
  • The rightmost cages in rows #3 and #4 are changed to contain floatie viruses instead of normal viruses.
  • The third cage in row #3 and the first cage in row #4 are changed to contain exploders instead of normal viruses.
  • The first Brick Block in row #6 now contains a pop cannon while the third Brick Block in row #5 now contains an exploder.

This stage is vertically oriented, with a column of bubble viruses between empty blocks on the sides, and two columns of viruses in the middle, with the column's bottom halves caged.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 45 to 35.
  • The viruses in columns #4 and #6 are changed to have matching colors horizontally for the first five rows and vertically for the other rows. Of the viruses, the third and fourth viruses in both columns are changed to be inside bubbles.
  • Pop cannons are now contained in the second and the second-last Brick Blocks in the topmost row.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (47).

The last stage of the area 2, with more of the stage coming after enough is cleared. The first part is vertically-oriented, made up of columns of viruses on the sides, a column of ice viruses in the middle, and sets of blocks between. The second part is filled with brick blocks, with viruses between them and a skill charger. Completing this stage earns the player a free Staff Ticket.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 4 lines are introduced.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 50 to 38.
  • All Empty Blocks are replaced with Brick Blocks.
  • The middle column of viruses including the floatie virus are no longer frozen.
  • The bottom 6 viruses in each of the leftmost and rightmost columns are changed to be inside bubbles.
  • A shell and a virus now occupy the spaces in the topmost row before the stage scrolls.
  • Exploders replace the first and last Brick Blocks in the first-revealed row.
  • A skill charger replaces the exploder in the third-revealed row.
  • The row of Empty Blocks are removed from the topmost row, rendering the stage with only 4 rows to scroll through instead of 5.

Within an 80 second time limit, the objective of this stage is to eliminate all viruses (44). The only color available in this stage is blue.

The stage is vertically-oriented. On the sides are columns of caged viruses with ice viruses above and below them. In the middle is two viruses surrounded by blocks, with a half-frozen lock block in the center. There are two exploders surrounded by Brick Blocks. All Brick Blocks contain hidden viruses.

Prior to version 2.0.0, the stage layout was different. The following changes were made:

  • All viruses except for the bottom-most row of viruses and the lock block are no longer half-frozen.
  • Two additional pink viruses are added between the pink viruses in the second row.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Timer is reduced from 100 seconds to 80 seconds.
  • The available color is changed to be blue only.
  • The viruses surrounded by Brick Blocks are replaced with exploders.
  • The viruses in the three topmost rows are changed to half-frozen.
  • All Empty Blocks are replaced with Brick Blocks with hidden viruses.

Starting from version 1.2.0, completing this stage awards the player 500 coins.

Within a 100 second time limit, the objective of this stage is to eliminate all viruses (16) and destroy all Brick Blocks (25). The only colors available in this stage are blue and pink.

There are two rows of floatie viruses each with a shell on the left of them, and above the two rows are Brick Blocks arranged in the shape of a graph.

The timer is increased from 80 seconds to 100 seconds since version 2.0.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The available colors are changed to pink and light-blue only.
  • The objective of this stage is changed to require destroying all Brick Blocks in addition to eliminating all viruses.
  • The floatie viruses now occupy the 8 rightmost spaces in rows #9 and #10 instead of being in 4 leftmost and 4 rightmost spaces in row #10. In addition, shells are added to the leftmost of both rows.
  • All Empty Blocks are replaced with Brick Blocks.

Starting from version 1.2.0, completing this stage awards the player 500 coins.

Within an 87 second time limit, the objective of this stage is to eliminate all viruses (53). The colors available in this stage are blue, green, and pink.

The stage is made almost entirely out of brick blocks, with only 6 viruses out in the open, 3 of them being fully-frozen. Every brick block contains either a virus or an exploder, or in the case of the third Brick Block in row #2, a shell.

The timer is increased from 57 seconds to 87 seconds since version 2.0.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Certain Brick Blocks have their contents changed from viruses to exploders: 2 in row #2 (#1, #4 and #7), 3 in row #3 (#3, #5 and #8), 2 in row #5 (#1 nd #5), 1 in row #6 (#8) and 3 in row #7 (#3, #5 and #7).
  • The virus hidden in Brick Block #3 in row #2 is replaced with a shell.

Completing this stage awards the player 1000 coins and a Staff Ticket (3 Hearts prior to version 1.2.0).

This stage has brick blocks and empty blocks arranged like a checkerboard, with floatie viruses on the top and bottom. Most of the brick blocks contain hidden viruses, except for the ones in the first row and the eighth row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 4 lines are introduced.

Completing this stage awards the player 1000 coins, and as of Version 1.2.0, Dr. Bowser Jr. is also awarded.
