World 12 is a candy-themed world in Dr. Mario World. The first area is available as of 2 AM PT on Feb. 3, 2020[1], while the second area, challenge stages and special stage are available as of 11 PM PT on Feb. 19, 2020[2].

When an area is cleared, Ant Troopers and Stingbies appear. The viruses featured in the regular stages are red, green and light-blue.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (32) and collect all keys (1).

The middle column has the key and door, separated by Brick Blocks. On the left and right sides, viruses are found with dust viruses above them, with Brick Blocks and Empty Blocks on the left and right columns on each side.

Starting with 42 capsules, the objective of this stage is to collect all virus coins (10).

In this stage, crates surround the red and light-blue lock blocks, with a green lock block in the middle of the stage. Additionally, colorful crates surround viruses on two or three sides at the top and bottom rows, each in front of a virus coin. The cages hold additional viruses along with ice fans, along with four larger virus coins behind some of the viruses.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (45).

In this stage, barrels are located in the middle of the stage in a "+" formation and diagonally toward the center. The former group are viruses while the latter group are exploders. Dust viruses in the shape of a diamond are found surrounding the middle barrels, containing viruses and exploders. Additional viruses surround the barrels.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (10), collect all coins from Coin Blocks (24) and break all barriers (21).

This stage contains colorful crates on the leftmost two columns and the rightmost two columns, along with barriers in the middle three columns and four surrounding flasks in the corners. The topmost Brick Blocks contain hidden viruses while the rest contain coins. Additional viruses are found along the two bottom-most row containing crates. The filled flasks in this stage will shoot out either viruses or exploders.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (39).

This stage has both viruses and Capsule Plus capsules occupying the spaces between crates. Some viruses, some crates and all Capsule Plus capsules are fully-frozen. Capsule Plus numbers can decrease even though it is frozen. A shell is in row #9 with two pop cannons, and an exploder is located in the middle of row #3.

The capsule amount is increased from 22 to 25 since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The first and middle viruses in row #1 are replaced with shells (the middle one still being fully frozen).
  • The middle virus in row #3 is replaced with an exploder.
  • A pop cannon, a shell and another pop cannon are added in the leftmost, middle and rightmost of row #9 respectively.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (38) and collect all keys (17).

In this stage, four rows of barrels with viruses in between each of the two rows are in the way of the keys and doors. The barrels in this stage contain one key, one lock and two viruses in each column, except for the middle column where only viruses are found.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (15) and break all barriers (28).

In this stage, barriers, viruses, lock blocks and Capsule Plus capsules are found in bubbles. Brick Blocks are found separating viruses, lock blocks and Capsule Plus capsules in groups of two, two of them behind cages. Barriers in bubbles will not change state until it pops. Pop cannons are located in the leftmost and rightmost columns.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The bubbles containing barriers in row #9 are replaced with pop cannons.

Starting with 43 capsules, the objective of this stage is to eliminate all viruses (12) and collect all purple coins (12).

In this stage, purple coins are found in the top row above floatie viruses, with additional floatie viruses near caged purple coins in bubbles, which are next to crates (each requires one hit to break).

Purple coins are unlocked in the following sequence:

  1. The 4 coins in row #1.
  2. The 4 coins in bubbles initially in row #6.
  3. The 4 coins in bubbles initially in row #8.

Starting with 39 capsules, the objective of this stage is to eliminate all viruses (32) and collect all coins from Coin Blocks (24).

In this stage, Brick Blocks form a pinwheel, with additional viruses shrouded in clouds. Of the Brick Blocks, eight contain coins, four near the middle contain ice fans, while the rest contain viruses.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 38 to 39.
  • The Brick Blocks that contain viruses near the middle are no longer there, revealing viruses at the top and bottom from the middle. The viruses on the left and right from the middle are shifted one space away from the middle.

Starting with 30 capsules, the objective of this stage is to collect all keys (2).

In this stage, the keys are found at the leftmost and rightmost columns. Floatie viruses in bubbles are found, with most of them on the leftmost and rightmost columns. All Brick Blocks except those found in the second-from-right column contains either shells or exploders. The filled flasks in this stage will shoot out either shells or exploders.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (60).

This stage contains lock blocks shrouded in clouds each above a crate and three bubble viruses and three spaces below exploders in cages, with each exploder surrounding fully-frozen viruses. Barrels are found in columns not containing the bubbles, and they all contain viruses. Additional viruses are found below the barrels.

Starting with 35 capsules, the objective of this stage is to collect all keys (3).

This stage is shaped like two keys formed by Brick Blocks. The bow of each key formation is formed by cages, with a hidden lock block found in the middle Brick Blocks. Shells are found in the Brick Blocks in the same row as the lock block at the bit of each key. The other Brick Blocks not initially in cages have hidden viruses and floatie viruses. Pop cannons are located in columns #3 and #7.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Pop cannons are added in the third and seventh spaces in row #10.

This stage is formed by four quadrants, each with barriers and viruses that cover each virus coin. The bottom-left quadrant ones are covered in dust while the bottom-right quadrant is covered in clouds. A shell is found in the middle of the stage, also covering a virus coin.

Starting with 15 capsules, the objective of this stage is to eliminate all viruses (4) (including any that appear from flasks before this objective is achieved) and break all Brick Blocks (36).

This stage contains Brick Blocks next to viruses and Capsule Plus capsules, where the Brick Blocks found in the top six rows are half-frozen. The filled flask in this stage will shoot out either viruses or Capsule Plus.

Starting with 60 capsules, the objective of this stage is to collect all coins from Coin Blocks (60).

This stage is shaped like a "W" formed by Brick Blocks and colorful crates. Barriers, both normal and in bubbles, are found in both halves. All Brick Blocks contain coins.

The capsule amount is increased from 44 to 48 since version 2.1.0. Besides that, this stage is otherwise the same.

Starting with 40 capsules, the objective of this stage is to collect all keys (8).

In this stage, all lock blocks are found in the topmost row, with two of them blocked by doors and another next to cages that require at least one of the other lock blocks cleared in order to properly match the lock block. In the same columns as the doors and lock blocks, bubble viruses are in the way of the keys. Additional half-frozen viruses are found in the second, middle and eighth columns.

In this stage, a ring of exploders in cages are surrounded by barrels, with the lock block found in the bottom row. All barrels contain viruses. The Brick Blocks in the bottom-most row, topmost row and the sides contain hidden ice fans.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (43). The only colors available in this stage are red and light-blue.

In this stage, diagonal lines of viruses and barrels are found, along with a few Capsule Plus capsules. Most viruses and all Capsule Plus capsules are encased in bubbles. All dust and barrels contain viruses.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The available colors are changed to red and light-blue only.

Starting with 40 capsules, the objective of this stage is to collect all virus coins (16).

In this stage, there are cages holding floatie viruses, with its corresponding lock block found in the barrel. Additional floatie viruses are found in clouds. Half-frozen viruses are found in the row below the barrel, with some ice fans found above and below them. Virus coins are found behind the half-frozen viruses and scattered among where the cages are. Exploders are found surrounded by crates and cages. The filled flasks in this stage shoot out either exploders or floatie viruses.

The last stage of the first area and starting with 50 capsules, the objective of this stage is to eliminate all viruses (67) (including any that appear from flasks before this objective is achieved) and collect all keys (7).

In this stage, the bottom four rows contain rows of viruses (normal, within barrels, half-frozen) with a gap in between each of them, and the five rows above it have viruses contained in all undamaged barrels and most Brick Blocks. Two keys and three exploders are found in a few of the Brick Blocks. Followed by this, the five undamaged barrels in a row all contain keys, along with six doors directly above them.

In the section above the doors, colorful crates are arranged in a pattern where floatie viruses and Capsule Plus are found in the larger gaps, while the lock blocks are found in the smaller gaps that unlock the colorful crates in the leftmost column, which are located below the door. The filled flasks in this stage will shoot out either viruses or viruses shrouded in dust.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 11 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Ant Trooper assistant upon completing this stage.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 43.
  • The stage is shifted up by one row, resulting in the scrolling lasting for 11 rows instead of 10 rows.

The capsule amount is increased from 43 to 50 since June 2021. Besides that, this stage is otherwise the same.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (59). The only color available in this stage is red.

In this stage, barrels are found surrounding viruses in bubbles and two exploders. The barrels in the same row as the bubbles contain floatie viruses except for the fifth barrel in row #7, which contain a normal virus. The other barrels contain normal viruses.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The available colors is changed to red only.
  • The contents of the barrels within rows #3 to #7 are changed from viruses to floatie viruses, except for the fifth barrel in row #7 (which retains the normal virus).

In this stage, colorful crates form an outline of a diamond, and viruses are found near the outline of colorful crates. A barrel containing a virus is in the middle of the stage, connected by colorful crates on the west and south and by floatie viruses on the north and east. Crates form a border around the structure.

In this stage, the 7x7 formation is occupied by viruses and four fully-frozen Brick Blocks (each containing coins) near the center. Crates are found on the left and above the formation and pop cannons are found below the formation with a shell in the same row as them.

The capsule amount is increased from 40 to 43 since version 2.1.0. Besides that, this stage is otherwise the same.

The filled flasks in this stage will shoot out exploders.

  1. The coins found within columns #1, #2, #8 & #9 and rows #2, #3, #7 & #8, for a total of 8 coins.

The Brick Blocks found near the green lock block and the Brick Blocks in the topmost row contain coins. The other Brick Blocks contain hidden viruses.

Purple coins are unlocked in the following sequence:

  1. All purple coins initially found in the green and light-blue cages, for a total of 22 coins.
  2. The 4 coins in row #9.

The capsule amount is increased from 37 to 40 since version 2.1.0. Besides that, this stage is otherwise the same.

In this stage, the are nine sections arranged in 3x3, each having a 3x3 formation. Each section in the corners and the middle contain half-frozen viruses surrounding a colorful crate. The other sections contain colorful crates surrounding exploders covered in dust, with one side open.

The capsule amount is increased from 35 to 38 since version 2.1.0. Besides that, this stage is otherwise the same.

Starting with 43 capsules, the objective of this stage is to eliminiate all viruses (8), break all barriers (22) and collect all keys (2).

In this stage, the leftmost and rightmost columns have colorful crates between the keys and doors. Shells are also found between barriers, with two shells in line with an Empty Block and thus are obstructed by it. Two rows of four viruses are confined with one side open, with one side in Empty Blocks and one side with both Empty Blocks and crates.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 43.
  • The crates in the eighth row have reduced durability (4 hits to 3 hits) while the remaining crate instead have greatly reduced durability (4 hits to 1 hit)

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 43 to 40.

In this stage, rows of barriers and a lock block are between the key and the door. Crates are found in the row below the door and the row above the key where it initially appeared.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 39 to 41.
  • The middle ice fan in column #7 is changed to become caged.

In this stage, crates and viruses are arranged in a checkered pattern, with some viruses encased in bubbles, shrouded in clouds, half-frozen, fully-frozen and covered in dust. Virus coins that are sized 3x3 are found behind all but the center of the formation.

In this stage, barriers are in the formation of brackets, and barrels containing viruses are found within and below it. Exploders are located in the topmost row.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (25) and collect all coins from Coin Blocks (33).

This stage is shaped like a flower in a pot. The topmost row of Brick Blocks contain hidden viruses, three of them floatie viruses. With the exception of the three Brick Blocks in the middle, the other Brick Blocks contain coins. The filled flask in this stage will shoot out Brick Blocks.

The capsule amount is increased from 35 to 38 since version 2.1.0. Besides that, this stage is otherwise the same.

  Stage 480

In the final stage of this world and starting with 40 capsules, the objective is to eliminate all viruses (30) and collect all purple coins (13).

In the beginning portion of this stage, barrels are found next to fully-frozen viruses, each containing colorful crates. Above the section with barrels are lines of viruses between colorful crates occupying two rows. Above that are two rows containing lines of floatie viruses between crates. In the section above, purple coins and viruses are found between colorful crates. More purple coins are found two rows above that section.

Purple coins are unlocked in the following sequence:

  1. The 3 coins in the first-revealed row.
  2. The 3 coins in the second-revealed row.
  3. The 3 coins in the third-revealed row.
  4. The 4 coins in the sixth-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 13. Upon completion, players earn a free Staff Ticket.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • The stage is shifted up by one row, resulting in the scrolling lasting for 8 rows instead of 7 rows.

Within an 80 second time limit, the objective of this stage is to break all Brick Blocks (46). The colors available in this stage are green, light-blue, and pink.

In this stage, Brick Blocks are found surrounding crates on the top half, the bottom-left quarter and bottom-right quarter. Separating them are fully-frozen viruses, with an empty mid-point. The top-left and top-right Brick Blocks contain shells.

Completing this stage awards the player 500 coins.

The timer is increased from 70 seconds to 80 seconds since version 2.1.0. Besides that, this stage is otherwise the same.

Within a 70 second time limit, the objective of this stage is to break all barriers (41). The colors available in this stage are green, light-blue, and pink.

In this stage, barriers are found in the leftmost columns and rightmost columns, along with the first, fourth and seventh rows. Viruses in groups of two rows by seven columns are found below each row of barriers.

Completing this stage awards the player 500 coins.

The timer is increased from 60 seconds to 70 seconds since version 2.1.0. Besides that, this stage is otherwise the same.

Within an 85 second time limit, the objective of this stage is to eliminate all viruses (62) and break all colorful crates (55). The colors available in this stage are green, light-blue, and pink.

In this stage, colorful crates have lines of viruses (horizontally and vertically) next to them. Colorful crates do not need a match of the same color to remove the ice, but still require a match of the same color to break. Additionally, a key is found in the middle column, which will be used to unlock a door between a row of fully-frozen green viruses. A row of fully-frozen colorful crates are above the aforementioned row. In the last two revealed rows, half-frozen colorful crates with like-colored viruses in between are found in two of the rows above it.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and a Staff Ticket.

The timer is increased from 80 seconds to 85 seconds since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The timer is reduced from 85 seconds to 80 seconds.
  • The colorful crates in rows #1, #2, #6 and each of the next three revealed rows are no longer frozen.
  • The viruses in row #3 are changed to become fully-frozen.
  • The colorful crates in the sixth- and seventh-revealed row are changed to become half-frozen, and like-colored viruses are added in between those colorful crates.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (63) and collect all coins from Coin Blocks (15).

In this stage, a lot of dust are found and contain viruses and crates, along with one shell, three Empty Blocks and five Brick Blocks each containing coins in the higher portion of the stage after it is scrolled up enough. A lock block is found initially and unlocks some crates and viruses.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Larry.
