That's easy. First, you copy the code of either an official SMW userbox or ask someone for theirs. Take mine, for example;

You copy the code, which is this;

Oh, and I forgot to mention, if you get more userboxes, paste their codes before the {{userboxbottom}} code, but after the {{userboxtop}} code.
