The fog urn (also simply referred to as the urn) is an ancient urn that contains the Phantasmal Fog, which is used by Bowser and Dr. Eggman in the story mode of the Nintendo 3DS version of Mario & Sonic at the London 2012 Olympic Games. The urn is a pale color and has a thin neck with larger base, with black swirl patterns at the bottom. The urn also has a separate lid, which is a rainbow-colored ball with a silver zig-zag patterned section leading into a red and yellow stopper.

The urn was originally located at a ruin site, though around 300 years before the events of the game was stolen by the pirate William Kidd, and magically sealed to keep the Phantasmal Fog inside to be able to remove it from the ruins. The Phantasmal Fog would form imposters of whoever the person holding the urn was thinking of as a curse to deter looters. The urn was later recovered from Kidd and was put on display by the British Museum until it was stolen shortly before the game's events. It was sold to the antique shop that owed Wario money, and was given to him to settle the debt by the owner who claimed that it was magic. Wario gave the urn to Dr. Eggman after losing a match to Metal Sonic, who then took it to his lab in Big Ben and designed the fog amplifying system and a way to break the seal. Bowser had Magikoopa use Stonehenge's latent energy and his magic to fully break the urn's seal, and then Dr. Eggman had Omega transport it back to him in Big Ben. He then used the urn to create the Phantasmal Fog to attempt to stop the Olympic Games.

In the story mode, the fog urn first appears in the night before, where Dr. Eggman places it in the fog amplifying system to produce Phantasmal Fog. Tails finds out information about it in The Secret of the Fog, where he finds a book on archaeology that Dr. Eggman had borrowed that details some of the urn's history, as well as finding out that it was recently stolen and theorising that Dr. Eggman managed to get hold of it somehow. It appears within the fog amplifying system again in Clear Skies Again, and when Omega mentions that he remembers being sent to Stonehenge in A Rainbow In Fog, Tails and Omochao theorise that the urn may have some connection to the site. In To the Ruins!, Mario, Luigi, Toad, Sonic, Tails and Omega encounter Magikoopa and Lakitu searching for the urn's lid, and more of Omega's memory banks are restored, causing him to remember transporting the urn as well as more of Dr. Eggman's actions with it. Omega also helps the group locate the urn's lid, which Tails takes with him to reseal the urn. After defeating Bowser and Dr. Eggman in The Grand Showdown, Mario and Sonic remove the urn from the fog amplifying system and use it to clear the Phantasmal Fog before resealing it with its lid. In An Old Urn, Wario gives the urn to Dr. Eggman after being defeated by Metal Sonic and explains how he got hold of it, and Dr. Eggman has Metal Sonic bring it to his lab in Big Ben to perform tests on it. In The Fog Returns, Dr. Eggman inspects the urn and use the fog amplifying system to partially break the urn's seal, producing the Phantasmal Fog. After defeating the imposter of Sonic, Dr. Eggman explains the history of the urn to Metal Sonic, Orbot & Cubot, as well as revealing that the urn must be fully opened for him to be able to use the fog at will.
