A Belly Blech is an infectious worm in Mario & Luigi: Superstar Saga and Mario & Luigi: Superstar Saga + Bowser's Minions. If the worm is ingested by someone, they will have their strength, size, and muscle mass dramatically increased, but they become mindlessly enraged and attack anything in sight.

To cause mayhem, Cackletta makes Queen Bean eat a Belly Blech worm, infecting her and allowing Cackletta and Fawful to steal the Beanstar. After Mario and Luigi defeat Queen Bean, Lady Lima instructs them to go to the Chucklehuck Woods to get Chuckola Reserve, a drink that can remove ingested Belly Blech worms. After giving the drink to Queen Bean, she spits out the worm and returns to normal.
