World 31 is the last world in Dr. Mario World, and is set on a stormy clifftop. The first area is available as of 12 PM PT on Jul. 28, 2021[1], while the second area, challenge stages and special stage are available as of 11 PM PT on Aug. 11, 2021.

Unlike the rest of the worlds, no familiar Mario-related species appear when an area is cleared, but instead only plants and crystals appear. The viruses featured in the regular stages are red, blue and yellow.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (9) and collect all virus coins (48). The only color available in this stage is red.

In this stage, there are four red viruses each surrounded by Empty Blocks on most sides and a Brick Block on one, and surrounding the groups of blocks are ice cubes. Floatie viruses occupy the sides and the middle of the gaps with the ice cubes, while rotating cannons are found in the corners. The Brick Blocks contain exploders. Virus coins are found behind every virus, floatie virus, rotating cannon, and most ice cubes. The only ice cube that doesn't have a virus coin is in the middle of the seventh row.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (54) and collect all purple coins (30).

In this stage, viruses are arranged in a checkered pattern, where purple coins are found in the gaps in the middle three columns, caged coins in the second and eighth columns, and bubbles in the other columns. The viruses starting from the second-revealed row are covered in ice. The topmost row contains a row of Empty Blocks.

Purple coins are unlocked in the following sequence:

  1. The 18 coins in columns #4 to #6.
  2. The 12 coins in columns #2 and #8.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (64) and collect all keys (1). The only colors available in this stage are blue and yellow.

In this stage, the middle column consists of crates in between the key and the door, in which the key is in the seventh row and the door is in the last-revealed row. Within the other columns, rows #5 and #6 contain viruses, rows #3 and #4 contain colorful crates, rows #1 and #2 contain viruses covered in ice, the first- and second-revealed rows contain barriers, the third- and fourth-rows contain viruses covered in ice, the fifth- and sixth-revealed rows contain rotating cannons, a flask and a shell handy bottle (the latter two are medium-sized), and the last two revealed rows contain floatie viruses. The filled flask in this stage will shoot out colorful crates.

This stage will scroll for two lines each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Starting with 40 capsules, the objective of this stage is to eliminate all viruses (18) and collect all virus coins (36). The only available color in this stage is red.

In this stage, there are four phases where each of them has 9 virus coins. The first phase contains virus coins each covered in the middle by either viruses encased in ice or exploders, in addition to Brick Blocks which cover the top row and the right column for each coin. The second phase contains virus coins covered in the middle by either floatie viruses encased in ice or exploders, where the virus coins covered by exploders are also covered by colorful crates at the left columns and the bottom rows.

The third phase contains virus coins covered in the middle by either barrels or shells in the middle, where each virus coin has crates in the rows above and the columns to the right of them. The top-left and bottom-right barrels contain Capsule Plus, the middle-right barrel contains a virus, while the rest contain exploders. In the fourth and final phase, each virus coin is covered by either a virus or floatie virus, where either virus type have Empty Blocks below them. Brick Blocks are found in the first, fourth, and seventh columns.

This stage will scroll through 9 rows simultaneously after all the virus coins are collected each time, and will scroll thrice this way.

Starting with 36 capsules, the objective of this stage is to eliminate all viruses (16) and collect all purple coins (20). The available colors in this stage are red, blue, yellow, and green.

In this stage, there are Capsule Plus in cages in groups of four, and their respective lock blocks surround an exploder in the middle. Since the lock blocks are covered by like-colored cages, the cages cannot be unlocked, and for the cages containing Capsule Plus to disappear, the number on them has to reach zero. Purple coins (caged and uncaged) are located in the middle where they surround the lock blocks. All the Brick Blocks and most of the barrels contain viruses, while the bottom-most barrel contain electric exploder. There are 36 full-sized rainbow capsules to start with, before normal capsules start to appear.

Purple coins are unlocked in the following sequence:

  1. The leftmost coin in row #6.
  2. The leftmost coins in rows #4 and #5, for a total of 2 coins.
  3. The two leftmost coins in row #3, for a total of 2 coins.
  4. The third and fourth coins in row #3, for a total of 2 coins.
